目的 观察血液透析联合血液灌流对维持性血液透析患者微炎症状态的影响.方法 选取我院行维持性血液透析1年以上的患者42例,随机分为血液透析组(HD组)21例、血液透析联合血液灌流组(HD+HP组)21例.HD组采用常规透析治疗每周3次,每次4 h;HD+HP组行血液透析治疗2周5次,每次4h,血液透析联合血液灌流2周1次,每组分别观察6个月,比较两组治疗前及治疗6个月后白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、血红蛋白(Hb)、血白蛋白(Alb)、SGA评分情况.结果 两组治疗前各项指标差异均无统计学意义(P＞0.05).两组治疗后:组内比较显示CRP、IL-6、TNF-α、Alb比治疗前均有改善,差异有统计学意义(P＜0.05);组间比较显示观察组治疗后CRP、IL-6、TNF-α、Alb、Hb、SGA评分改善情况优于对照组,差异有统计学意义(P＜0.05).结论 HD+ HP治疗可显著改善维持性血液透析患者微炎症状态及营养状态,提高患者生活质量.

목적 관찰혈액투석연합혈액관류대유지성혈액투석환자미염증상태적영향.방법 선취아원행유지성혈액투석1년이상적환자42례,수궤분위혈액투석조(HD조)21례、혈액투석연합혈액관류조(HD+HP조)21례.HD조채용상규투석치료매주3차,매차4 h;HD+HP조행혈액투석치료2주5차,매차4h,혈액투석연합혈액관류2주1차,매조분별관찰6개월,비교량조치료전급치료6개월후백세포개소-6(IL-6)、종류배사인자-α(TNF-α)、C반응단백(CRP)、혈홍단백(Hb)、혈백단백(Alb)、SGA평분정황.결과 량조치료전각항지표차이균무통계학의의(P＞0.05).량조치료후:조내비교현시CRP、IL-6、TNF-α、Alb비치료전균유개선,차이유통계학의의(P＜0.05);조간비교현시관찰조치료후CRP、IL-6、TNF-α、Alb、Hb、SGA평분개선정황우우대조조,차이유통계학의의(P＜0.05).결론 HD+ HP치료가현저개선유지성혈액투석환자미염증상태급영양상태,제고환자생활질량.
Objective To observe the effect of hemodialysis combined with hemoperfusion on microinflammatory state in maintenance hemodialysis patients.Methods In our hospital tmderwent maintenance hemodialysis for more than 1 years in 42 patients,were randomly divided into hemodialysis group (HD group) 21 cases,Hemoperfusion combined with hemodialysis group (HD + HP group) 21 cases.HD group was treated with conventional dialysis 3 times a week,4 hours each time.HD + Hp group underwent hemodialysis for 5 times in 2 weeks,4 hours each time.Hemoperfusion combined with hemodialysis 1 times in 2 weeks.Each group were observed for 6 months,comparison of the two groups before treatment and after 6 months of interleukin-6(IL-6),Tumor necrosis factor alpha (TNF-alpha),C reactive protein (CRP),Hemoglobin (Hb),Serum albumin (Alb),SGA score.Results The two groups before treatment,there were no significant differences in each index (P ＞ 0.05).The two groups after treatment:within group comparisons showed that CRP,IL-6,TNF-alpha,Alb than before treatment were improved.The difference was statistically significant(P ＜ 0.05).Comparison between groups showed that the observation group after treatment CRP,IL-6,TNF-alpha,Alb,HGB,SGA was better than that in control group.The difference was statistically significant (P ＜ 0.05).Conclusion HD + HP treatment can significantly improve the maintenance hemodialysis patients with micro inflammatory state and nutritional status,improving the quality of life for patients.
