目的：探讨短期外用糖皮质激素时联用抗菌药物的必要性。方法：采用自身对照的临床研究，对47例寻常型银屑病和皮炎湿疹类皮肤病患者每天2次外用0．02％丙酸氯倍他索乳膏，在外用前和连续外用7天时对用药部位进行细菌培养，观察皮肤是否出现感染并分析用药前后的皮肤菌群变化。结果：47例患者均未出现皮肤局部感染表现，患者皮肤在用药前或／和用药第7天检出金黄色葡萄球菌的病例共23例，其中菌落数量增加或保持不变的15例，数量减少的8例。未见溶血性链球菌及其它致病菌生长。用药前后皮肤菌群的差异无统计学意义。结论：不推荐在非感染性皮肤病外用强效糖皮质激素时联合使用抗菌药物。
목적：탐토단기외용당피질격소시련용항균약물적필요성。방법：채용자신대조적림상연구，대47례심상형은설병화피염습진류피부병환자매천2차외용0．02％병산록배타색유고，재외용전화련속외용7천시대용약부위진행세균배양，관찰피부시부출현감염병분석용약전후적피부균군변화。결과：47례환자균미출현피부국부감염표현，환자피부재용약전혹／화용약제7천검출금황색포도구균적병례공23례，기중균락수량증가혹보지불변적15례，수량감소적8례。미견용혈성련구균급기타치병균생장。용약전후피부균군적차이무통계학의의。결론：불추천재비감염성피부병외용강효당피질격소시연합사용항균약물。
Objective:To investigate the effects of short-term topical potent glucocorticoid on skin flora of patients with non infective skin diseases.Methods:Topical 0.02%Clobetasonl Propionate Cream twice daily were adopted on 47 patients suffering from psoriasis or eczema dermatitis.Clinical and microbiological evaluation were scheduled for days 0, and 7 at the identical location.Results: No infection happened on any patient.Staphylococcus aureus were detected in 23/47 (48.9%) cases before or /and using drug for sev-enth days, of which the number of colonies increased or unchanged 15/47 (31.9%) cases, decreased 8/47(17.0%) cases .No he-molytic streptococcus, fungi and other pathogens.There was no statistically significant difference of skin flora between before and after treatment.Conclusions:There is no significant effect of short-term topical potent glucocorticoid on skin flora of patients with non in-fective skin diseases.
