目的:观察拉莫三嗪添加-替换疗法对小儿难治性癫痫患儿的疗效。方法选取近年来收治的患儿100例,按照临床分型分为大发作组、复杂组、单纯组、小发作组,均行拉莫三嗪添加-替换疗法治疗,并根据患儿平均每月癫痫的发病次数及改善情况评估用药效果。结果治疗总有效率最高的是小发作组,其次是复杂组,而大发作组和单纯组则较低。各组患儿治疗后3,6个月的发病次数较治疗前均有明显下降( P <0.05)。结论对于丙戊酸应用无效的小儿难治性癫痫患儿,使用拉莫三嗪添加-替换疗法可在增强原治疗效果的基础上减少不良反应的发生,更好地保护神经系统功能,预后良好,可临床推广。

목적:관찰랍막삼진첨가-체환요법대소인난치성전간환인적료효。방법선취근년래수치적환인100례,안조림상분형분위대발작조、복잡조、단순조、소발작조,균행랍막삼진첨가-체환요법치료,병근거환인평균매월전간적발병차수급개선정황평고용약효과。결과치료총유효솔최고적시소발작조,기차시복잡조,이대발작조화단순조칙교저。각조환인치료후3,6개월적발병차수교치료전균유명현하강( P <0.05)。결론대우병무산응용무효적소인난치성전간환인,사용랍막삼진첨가-체환요법가재증강원치료효과적기출상감소불량반응적발생,경호지보호신경계통공능,예후량호,가림상추엄。
Objective To observe the curative effect of lamotrigine added - replacement therapy of refractory epilepsy in children. Methods Selected 100 cases of refractory epilepsy in our hospital in recent years and divided into big attack group, complex group, simple group, small attack group underwent lamotrigine added - replacement therapy. Evaluated the effect of drugs according to the onset times and improve the situation of children with a monthly average of epilepsy. Results The overall clinical efficacy in children com-pared with various types of epilepsy, treatment was found the highest efficiency was a small attack group, the second group was com-plex, and the big attack group and single group were low. 3 months after treatment, treatment after the onset times of 6 months com-pared with before treatment were significantly decreased, the difference changes significantly. Conclusion For The invalid application of valproic acid in children with refractory epilepsy, lamotrigine added - replacement therapy can reduce the occurrence of adverse reaction, better in the foundation to enhance the original treatment effect on the protection of the nervous system function, prognosis is good, it is recommended for clinical application.
