目的:调查分析川东北地区某二甲医院超药品说明书用药的情况,为促进皮肤科合理用药提供基线数据。方法随机抽取2013年门诊皮肤科处方2400张,含6137条用药记录。根据药品说明书内容,判断是否超说明书用药,并分析超说明书用药的类型。结果超说明书用药处方423张,占17.62%;超说明书用药记录1152条,占18.77%。超说明书用药类型中居前3位的为超适应证(38.30%)、改变给药途径(26.71%)、超剂量(19.39%)。结论皮肤科门诊处方中超药品说明书用药情况普遍,应严格按照药品说明书规定的适应证、用法用量,选择适宜患者的药物剂型,以减少超说明书用药,提高用药安全性。

Objective To investigate the off - label drug use in outpatient department of dermatology in a Level 2A hospital of Northeast Sichuan, and to provide reference for promoting the rational drug use. Methods Author sampled 2 400 prescriptions in total, all sample prescriptions of it contained 6 137 records, randomly sampled the prescriptions in outpatient department of dermatology in 2013, and ac-cording to these relevant information based on drug instructions, to estimate whether it belongs to the off - label drug use, and the off -label used was determined according to the package inserts in the following aspects, the types of off - label use. Results There were 423(17. 62% ) off - label drug use prescriptions, involved 1 152 records(18. 77% ) about off - label use. Among them 38. 30% prescrip-tions were exceed indications, 26. 71% prescriptions were changed the route of administration, 19. 39% prescriptions were exceed the pre-scribed dose. Conclusion The situation of off - label drug use in department of dermatology is very common. It is an effective approach to reduce off - label use and improve drug safety through strictly abiding by the instruction of usage and dosage as well as selecting a suitable dosage form.
