目的探讨原发性肝癌患者血清酶水平变化及预后意义.方法收集苏州大学附属第三医院原发性肝癌患者术前血清样本147例、肝血管瘤患者33例、肝硬化患者36例、健康体检者79例,应用秩和检验比较谷丙转氨酶、谷草转氨酶、谷丙/谷草转氨酶比值、γ-谷氨酰转肽酶、碱性磷酸酶、乳酸脱氢酶和胆碱酯酶水平,分析谷丙转氨酶、谷草转氨酶与临床病理特征关系,Cox比例风险模型分析血清酶与患者预后关系.结果谷丙转氨酶、谷草转氨酶、谷丙/谷草转氨酶比值、γ-谷氨酰转肽酶、碱性磷酸酶、乳酸脱氢酶和胆碱酯酶在肝癌、肝血管瘤、肝硬化者及健康体检者中差异有统计学意义,碱性磷酸酶、乳酸脱氢酶是原发性肝癌的独立预后因素.结论血清酶在原发性肝癌的诊断和预后判断中具有重要价值.

Objective To investigate the variation of serum enzyme levels in primary hepatic carcinoma and their significances of prognosis. Methods Preoperative serum of 147 hepatic carcinoma, 33 hepatic hemangioma, 36 hepatic cirrhosis patients and 79 healthy individuals were collected. The rank sum test was used to compare the levels of GPT, GOT, GPT/GOT,γ-GT, ALP, LDH and ChE. The levels of GPT and GOT were compared with clinical pathology features. The Cox proportional hazards model was used to analyze the relationship between prognosis and serum enzyme levels. Results The levels of GPT, GOT, GPT/GOT,γ-GT, ALP, LDH and ChE were discrepant in different patient groups and the control group, with statistically significance. ALP and ChE levels were independent factors for the prognosis of primary hepatic carcinoma. Conclusion The levels of serum enzyme have important value in diagnosis and prognosis criterion of primary hepatic carcinoma.
