目的:探讨结直肠癌合并幽门螺旋杆菌(Hp)感染患者血中 I 型胶原氨基端前肽(PINP)、I 型胶原羧基端肽β特殊序列(β- CTX)的水平及临床意义。方法选择2010年2月至2014年12月期间70例结直肠癌患者为研究对象,70例结直肠癌中,临床分期 I ~ II 期48例,III ~ IV 期22例;低分化20例,中高分化50例;Hp 感染阳性者24例;Hp 感染阴性者46例。选择70例健康者为对照组。比较结直肠癌患者与对照组的 PINP、β- CTX 水平;不同分期、不同分化程度、Hp 感染与否以及促进骨形成药物治疗前后结直肠癌患者的 PINP、β- CTX 水平。结果结直肠癌组血清PINP、β- CTX 含量明显低于对照组( P ﹤0.01);III - IV 期结直肠癌患者血清 PINP、β- CTX 含量明显低于 I - II 期结直肠癌患者( P ﹤0.01);结直肠癌低分化组血清 PINP、β- CTX 含量明显低于结直肠癌中高分化组( P ﹤0.01);结直肠癌 Hp 阳性组血清 PINP、β- CTX 水平低于结直肠癌 Hp 阴性组( P ﹤0.01);结直肠癌患者经治疗后血清 PINP、β- CTX水平明显较治疗前提高( P ﹤0.01)。结论结直肠癌患者血中 PINP、β- CTX 低表达;PINP、β- CTX 的表达与结直肠癌的临床分期及分化程度有关,同时亦与结直肠癌患者 Hp 感染状况有关;给予结直肠癌患者促进骨形成药物治疗,可以提高患者的 PINP、β- CTX 水平,降低患者骨质疏松症发病风险。

목적:탐토결직장암합병유문라선간균(Hp)감염환자혈중 I 형효원안기단전태(PINP)、I 형효원최기단태β특수서렬(β- CTX)적수평급림상의의。방법선택2010년2월지2014년12월기간70례결직장암환자위연구대상,70례결직장암중,림상분기 I ~ II 기48례,III ~ IV 기22례;저분화20례,중고분화50례;Hp 감염양성자24례;Hp 감염음성자46례。선택70례건강자위대조조。비교결직장암환자여대조조적 PINP、β- CTX 수평;불동분기、불동분화정도、Hp 감염여부이급촉진골형성약물치료전후결직장암환자적 PINP、β- CTX 수평。결과결직장암조혈청PINP、β- CTX 함량명현저우대조조( P ﹤0.01);III - IV 기결직장암환자혈청 PINP、β- CTX 함량명현저우 I - II 기결직장암환자( P ﹤0.01);결직장암저분화조혈청 PINP、β- CTX 함량명현저우결직장암중고분화조( P ﹤0.01);결직장암 Hp 양성조혈청 PINP、β- CTX 수평저우결직장암 Hp 음성조( P ﹤0.01);결직장암환자경치료후혈청 PINP、β- CTX수평명현교치료전제고( P ﹤0.01)。결론결직장암환자혈중 PINP、β- CTX 저표체;PINP、β- CTX 적표체여결직장암적림상분기급분화정도유관,동시역여결직장암환자 Hp 감염상황유관;급여결직장암환자촉진골형성약물치료,가이제고환자적 PINP、β- CTX 수평,강저환자골질소송증발병풍험。
Objective To study PINP,beta CTX levels in serum of colorectal cancer with helicobacter pylori infection and clinical signifi-cance. Methods Between January 2012 and December 2014,70 cases of colorectal cancer patients were selected as the research object. Of 70 cases of colorectal cancer,the clinical stage I - II 48 cases,III - IV 22 cases;50 cases of 20 patients with poorly differentiated the high differ-entiation. Hp infection were found in 24 cases. Hp infection negative were 46 cases. 70 cases of healthy subjects were selected as control group. Comparison of colorectal cancer patients and the control group PINP,beta CTX level were performed. Different stages,different degree of differen-tiation,Hp infection,and promote bone formation PINP of colorectal cancer patients before and after drug treatment,beta CTX level were recorded and compared. Results Serum PINP,beta CTX content colorectal cancer group was obviously lower than the control group( P ﹤ 0. 01). III -IV colorectal cancer patients serum PINP,beta CTX content significantly lower than stage I - II colorectal cancer patients( P ﹤ 0. 01). Poorly differentiated group serum PINP,beta CTX content were significantly lower than the high differentiation group( P ﹤ 0. 01). H. pylori positive se-rum PINP,beta CTX level were lower than that of h. pylori negative group( P ﹤ 0. 05). Colorectal cancer patients after treatment serum PINP, beta CTX level was improved( P ﹤ 0. 01). Conclusion Colorectal cancer patientsˊblood PINP,beta CTX lower expression;PINP,beta of CTX expression and clinical staging of colorectal cancer and the differentiation degree,at the same time also is associated with colorectal cancer patients with Hp infection status. To promote bone formation,drug treatment in patients with colorectal cancer can improve the patientˊs level of PINP,beta CTX,reduce the risk of osteoporosis patients.
