目的:探讨超声乳腺影像报告和数据系统(BI-RADS-US)对乳腺良、恶性病灶的评估价值。方法回顾分析103例满足 BI-RADS-US 报告要求乳腺肿块患者的109个病灶,以术后病理诊断结果为金标准,计算 BI-RADS-US 诊断乳腺恶性病灶的准确性、敏感性、特异性、阳性预测值(PPV)及阴性预测值(NPV)。统计分析 BI-RADS-US 关于形态、方位、边界、边缘回声、后方回声、钙化等描述术语对良恶性病灶的鉴别价值。结果109个病灶的 BI-RADS-US 分级:良性50个,恶性59个;2~5级诊断恶性肿瘤的PPV 分别为0、5.8%、27.7%和93%。BI-RADS-US 诊断乳腺恶性肿瘤的的准确性、敏感性、特异性、PPV 和 NPV 分别为82.5%、96%、36%、93%和63%。在良恶性结节中,6个有统计学意义的超声描述词分别为:不规则形、非平行生长、边缘不光整,周围高回声声晕,后方回声衰减,内部微钙化;其 PPV 值分别为94%,87%,85%,95%,88%,84.6%。结论 BI-RADS-US 具有较高的诊断准确性,对预测乳腺良、恶性病灶有较高的价值,为临床提供了标准化和规范化的报告。

목적:탐토초성유선영상보고화수거계통(BI-RADS-US)대유선량、악성병조적평고개치。방법회고분석103례만족 BI-RADS-US 보고요구유선종괴환자적109개병조,이술후병리진단결과위금표준,계산 BI-RADS-US 진단유선악성병조적준학성、민감성、특이성、양성예측치(PPV)급음성예측치(NPV)。통계분석 BI-RADS-US 관우형태、방위、변계、변연회성、후방회성、개화등묘술술어대량악성병조적감별개치。결과109개병조적 BI-RADS-US 분급:량성50개,악성59개;2~5급진단악성종류적PPV 분별위0、5.8%、27.7%화93%。BI-RADS-US 진단유선악성종류적적준학성、민감성、특이성、PPV 화 NPV 분별위82.5%、96%、36%、93%화63%。재량악성결절중,6개유통계학의의적초성묘술사분별위:불규칙형、비평행생장、변연불광정,주위고회성성훈,후방회성쇠감,내부미개화;기 PPV 치분별위94%,87%,85%,95%,88%,84.6%。결론 BI-RADS-US 구유교고적진단준학성,대예측유선량、악성병조유교고적개치,위림상제공료표준화화규범화적보고。
Objective To explore the value of ultrasound breast imaging reporting and data system(BI-RADS-US)in evaluation of breast diseases. Methods A total of 109 breast lesions from 103 patients satisfying the criteria of BI-RADS were retrospectively analyzed with BI-RADS lexicon,and their postoperative pathological results were compared. The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of BI-RADS-US to diagnosis breast malignant lesions were calculated. According to the BI-RADS-US lexi-con,descriptive terms such as irregular shape,non-parallel orientation,non-circumscribed margin,echogenic halo,shadowing and micro-calcifications,were used to identify breast benign and malignant lesions via chi-square test. Results Of 109 breast lesions,50 were patho-logically diagnosed to be benign and 59 to be malignant. The positive predictive values of breast malignant lesions from grade two to five were 0,5. 8% ,27. 7% and 93% ,respectively,and the calculated accuracy,sensitivity,specificity,positive predictive value and negative predic-tive value were 82. 5% ,96% ,36% ,93% ,63%. Irregular shape,non-parallel orientation,non-circumscribed margin,echogenic halo,shado-wing and micro-calcifications,these six ultrasound descriptive terms were more frequently found in malignant nodules than in benign nodules, and their positive predictive value were 94% ,87% ,85% ,95% ,88% ,84. 6% . Conclusion BI-RADS-US classification has high application value in diagnosis of benign and malignant breast lesions because of its high diagnostic accuracy and standard reporting data.
