目的 提高外科实习医师临床技能,提高外科学教学水平.方法 选择上海交通大学2009级临床医学专业135名实习医师为研究对象,随机分为实验组和对照组.对照组采取传统教学方法,实验组采用包括标准化个体教学、递进式操作培训及充分参与临床实践等内容的规范化临床强化培训.外科实习结束后,对比分析实验组学生强化培训前后的徒手打结、器械缝合打结成绩,以及实验组与对照组学生的客观结构化临床考试成绩,从而评估强化培训的教学效果.结果 实验组学生经过强化培训后,徒手打结平均增加15个/分钟,器械缝合打结平均提高0.8个/分钟,培训前后学生成绩的差异具有统计学意义.实验组学生的客观结构化临床考试成绩明显高于对照组学生,成绩的差异具有统计学意义.结论 规范化临床强化培训能够显著提高外科实习医师临床技能.

Objective We explored to enhance the level of surgical education and to improve the clinical skills of the surgical interns.Methods 135 interns from Shanghai JiaoTong University specializing in clinical medicine in grade 2009 were randomized into control group and experimental group.The interns from the control group received the traditional training,and those from the experimental group received the standardized clinical intense training,which includes standardized individual teaching,progressive operation training and full participation in clinical practice.To assess the effectiveness of the intense training,the average scores of the experimental group in knot typing and suturing before and after training were compared,and the after-training objective structured clinical examination (OSCE) score of the experimental group was also compared with the control group.Results After training,the average score of the experimental group was increased by 15 pre minutes in knot typing and 0.8 per minute in suturing.And in the after-training OSCE,the experimental group performed better than the control group.The differences in scoring above were all statistically significant.Conclusions The standardized clinical intense training can significantly improve the clinical skills of the interns,and has promotional value.
