目的探讨体外膜肺氧合(ECMO)在心脏移植后右心衰竭治疗中的效果.方法回顾性分析我院2012年1月至2014年12月期间因心脏移植后出现右心功能衰竭而使用ECMO进行治疗的18例患者的资料.结果 18例患者中痊愈出院15例,气管切开11例,呼吸机辅助时间为(116.6±28.9)h,住重症监护病房时间为(23.1±12.6)d,手术后住院时间为(32.3±15.4)d,无患者出现溶血并发症,术后并发症肺部感染12例,急性肾功能不全12例,肝功能不全7例,消化道出血5例.结论 ECMO对于心脏移植后右心功能衰竭治疗效果良好,尽早开始使用可以提高治疗成功率.

Objective To study the efficacy of extracorporeal membrane oxygenation (ECMO) in the treatment of right ventricular failure after heart transplantation.Method The adjuvant efficacy of ECMO in the treatment of 18 patients with right heart failure after heart transplantation in our hospital during January 2012 to December 2014 was retrospectively analyzed.Result For the 18 cases,15 survived to discharge,11 received tracheotomy,the total mechanical ventilation time was 116.6 ± 28.9 h,intensive care time was 23.1 ± 12.6 days,and hospital stay was 32.3 ± 15.4 days.Postoperative complications included pulmonary infection in 12 patients,acute renal insufficiency in 12 patients,hepatic dysfunction in 7 patients,and gastrointestinal bleeding in 5 patients.No hemolytic complications occurred.Conclusion ECMO support for patients with right heart failure after cardiac transplantation has a good therapeutic efficacy.The early application of ECMO will improve the outcomes of treatment.Meanwhile,because of the high treatment risk,more experience can significantly reduce the incidence of complications.
