目的：分析影响初产妇1小时内开奶的相关因素,为促进及时有效地母乳喂养提供科学依据。方法对2014年7月1日-12月31日期间在珠海市某院住院待产分娩的孕妇的一般人口学特征、孕期情况和产前母乳喂养意向等进行问卷调查,于产妇分娩结束后半小时内再次调查其产后母乳喂养意愿并随访产后新生儿的开奶时间。对研究对象的一般情况和母乳喂养意向及喂养方式进行统计,通过多因素 Logistic 回归分析产后1小时内开奶的相关因素。结果随访到的454名初产妇中,1小时内开奶者为323人(71.2%)。自然分娩、母婴同室和产后纯母乳喂养意向为产后1小时内开奶的独立保护因素。结论应加强孕前指导,树立产妇自然分娩的观念和信心,做好围产保健,确保围产安全和母婴同室,并及时做好纯母乳喂养健康教育和技能指导。

Objectives To investigate and analyze the factors associated with breastfeeding initiation within 1 hour of birth among first-time mothers and put forward the policies about breastfeeding into effect.Methods Expectant women in hospital were selected and investigated using questionnaires regarding demographic,conditions of pregnan-cy and breastfeeding intention during July 1 to December 31 in 2014.Within half an hour after delivery their breast-feeding intention was investigated again with the time for breastfeeding initiation was noted.Results 323(71.2%) started to breastfeed within one hour after childbirth among the subjects.Natural delivery,rooming-in and exclu-sive breastfeeding intention were the protective factors for breastfeeding initiation within 1 hour after delivery for first-time mothers.Conclusions Help the puerpera to trust in natural delivery and prenatal health care should be emphasized to ensure the safety of mother and neonatal.Health education and skills regarding exclusive breastfeed-ing were suggested to provide for mothers with their family members in time.
