目的:检测 Twist、E-cadherin及 N-cadherin 在胃癌中的表达和临床意义,探讨 Twist介导的上皮间质转化(EMT)在胃癌发生、发展及转移中的作用。方法采用免疫组织化学技术分别检测 Twist、E-cadherin 及 N-cadherin 在108例胃癌原发灶及相应的癌旁组织中的表达,统计分析Twist、E-cadherin 及 N-cadherin 在胃癌原发灶表达强度的相关性及与胃癌病人临床病理特征之间的关系。结果① Twist 和 N-cadherin 在胃癌中的表达率分别显著高于相应癌旁组织(53.7%比38.0%,P =0.020;55.6%比33.3%,P =0.001);E-cadherin 在胃癌中的表达率显著低于相应的癌旁组织(41.7%比71.3%,P <0.001)。②胃癌原发灶中 Twist、E-cadherin 及 N-cadherin 的表达高低分别与 T 分期(P <0.001;P =0.005;P <0.001)、TNM 分期(P =0.005;P =0.029;P =0.016)及淋巴结转移(P =0.005;P =0.001;P =0.002)有关。③在胃癌原发灶中,Twist 的表达与 N-cadherin 的表达呈正相关(r =0.365,P <0.001);Twist 的表达与 E-cadherin 的表达呈负相关(r =-0.232,P =0.016)。④ Logistic 回归分析提示 Twist(P <0.001)和 N-cadherin(P <0.001)阳性表达分别是 T 分期的独立危险因素;E-cadherin 阴性表达是淋巴结转移的独立危险因素(P =0.001)。Cox 风险回归多因素生存分析显示,淋巴结转移(P =0.029)及 Twist 阳性表达(P =0.005)是影响胃腺癌病人术后生存的独立因素。结论在胃癌组织中,Twist 表达和 N-cadherin 表达上调,E-cadherin 表达下调, Twist 介导的 EMT 可能与胃癌的侵袭转移有关。

Objective To observe the clinical significances and the expressions of Twist,E-cad-herin and N-cadherin during Twist-mediated epithelial-mesenchymal transition (EMT)in gastric canc-er tissues.Methods The expressions of Twist,E-cadherin and N-cadherin were detected by immuno-histochemistry in primary cancer tissues and in normal mucosa tissues of 108 patients with gastric cancer.The correlation among expressions of Twist,E-cadherin and N-cadherin in gastric cancer tis-sues and the clinical pathologic characteristics were further analyzed.Results (1 )The positive ex-pression rates of Twist and N-cadherin in gastric cancer tissues were 53.7% and 55.6% respectively, which were higher than those in gastric normal mucosa tissues (38.0% and 33.3%)with significant differences (P =0.020;P =0.001);but lower positive expression rate of E-cadherin in gastric cancer tissues was found (41 .7%)compared with those in gastric normal mucosa tissues (71 .3%),signifi-cant difference could be seen between both groups (P <0.001);(2)The expressions of Twist,E-cad-herin and N-cadherin in primary gastric cancer tissues was noted to present tendency correlating with the T stage (P <0.001 ;P =0.005;P <0.001 ),pTNM stage (P =0.005;P =0.029;P =0.016) and lymph node metastasis (P =0.005;P = 0.001 ;P = 0.002);(3 )The expression of Twist was positively seen correlation with that of N-cadherin (r =0.365,P <0.001 ),and but with that of E-cadherin (r=-0.232,P =0.016)in primary gastric cancer tissues.(4)Logistic regression analysis revealed that T stage was an independent risk factor for positive expression of Twist (P <0.001 )or N-cadherin (P <0.001),and that lymph node metastasis was the independent risk factor for negative expression of E-cadherin (P =0.001).Multivariate analysis with the COX regression models showed that expression of Twist (P =0.005)and lymph node metastasis (P =0.029)were significantly corre-lated with the poor prognosis.Conclusions The expressions of Twist and N-cadherin were up-regula-ted,whereas the expression of E-cadherin was down-regulated in primary gastric cancer tissues,sug-gesting Twist-mediated EMT may be involved in the invasion and metastasis of gastric cancer.
