目的 探讨微小RNA(miRNA,miR)-192在Ⅱ、Ⅲ期结直肠癌组织中表达及其与患者术后复发转移的关系.方法 收集行结直肠癌根除术的Ⅱ、Ⅲ期患者新鲜标本119例,其中,3年内发生复发转移45例(转移复发组),未出现复发转移74例(未转移复发组).同时,取40例确证无癌细胞的癌旁组织作为对照组,利用反转录实时定量聚合酶链反应(PCR)检测肿瘤及癌旁组织中miR-192表达,分析其对结直肠癌患者术后复发转移的影响.结果 miR-192在结直肠癌患者肿瘤组织中相对表达量(0.27 ±0.06)显著低于癌旁组织中的(0.56±0.12),差异有统计学意义(P<0.05),miR-192在转移复发组患者肿瘤组织中相对表达量(0.13±0.04)显著低于未转移复发组(0.32±0.09),差异有统计学意义(P<0.05);miR-192高表达组患者术后复发率为29.5%,低于低表达组的61.3%,差异有统计学意义(P<0.05);转移复发组患者中miR-192高表达者术后中位无复发生存时间为18.3个月,长于低表达者的14.7个月,差异有统计学意义(P<0.05);单因素生存分析显示,miR-192表达降低与患者术后无复发生存时间缩短有关,且在Ⅲ期患者中更为明显(P<0.05),Cox比例风险模型分析显示,miR-192低表达[相对危险度(RR) =0.402,95%可信区间(CI):0.312 ~0.803,P<0.05]是影响结直肠癌患者术后无复发生存期的独立风险因素.结论 miR-192在结直肠癌中呈低表达,且与患者术后肿瘤复发转移密切相关,是影响结直肠癌术后无复发生存期的独立危险因素.

Objective To investigate the expression of microRNA (miRNA,miR)-192 in Ⅱ-and Ⅲ-stage colorectal cancer tissues and its relationship with postoperative recurrence and metastasis.Methods 119 cases of fresh specimens from patients with colorectal cancer of stages Ⅱ and Ⅲ undergoing colorectal cancer eradication in Henan Tumor Hospital were selected from January 2010 to December 2010.Forty-five cases had recurrence within three years (metastasis and recurrence group),and 74 cases had no recurrence (non-metastasis and recurrence group).At the same time,40 cases of adjacent non-cancerous tissues were selected as the control group.The expression levels of miR-192 in the tumor and adjacent non-cancerous tissues were detected by reverse transcription quantitative real-time polymerase chain reaction.Its impacts on postoperative recurrence of colorectal cancer patients after surgery were analyzed.Results The relative expression level of miR-192 in the tumor tissues of patients with colorectal cancer was (0.27 ±0.06),which was significantly lower than (0.56 ±0.12) in the adjacent non-cancerous tissues (P < 0.05).The relative expression level of miR-192 in the tumor tissues of the metastasis and recurrence group was (0.13 ±0.04),which was significantly lower than (0.32 ±0.09) in the non-metastasis and-recurrence group (P < 0.05).The recurrence rate in the high miR-192 expression group was 29.5%,which was significantly lower than the low miR-192 expression group (P < 0.05).The median relapse-free survival of the patients with high miR-192 expression in metastasis and recurrence group was 18.3 months,which was significantly longer than 14.7 months in the patients with low miR-192 expression (P < 0.05).Univariate analysis showed that miR-192 low expression was related with the reduction of postoperative recurrence-free survival time,more pronounced in patients with stage Ⅲ (P < 0.05).Cox proportional hazards model analysis showed that the low expression of miR-192 [relative risk (RR) =0.402,95 % confidence interval (CI):0.312-0.803,P < 0.05] was an independent risk factor influencing postoperative relapse-free survival of colorectal cancer patients.Conclusion miR-192 was lowly expressed in colorectal cancer,and was closely associated with postoperative recurrence of patients.It was an independent risk factor influencing postoperative recurrence-free survival of patients with colorectal cancer.
