目的 评价丙泊酚后处理对大鼠肝缺血再灌注时远隔脏器肾脏细胞B淋巴细胞瘤/白血病-2(Bcl-2),Bcl相关X蛋白(Bax)的影响.方法 健康雄性SD大鼠54只,随机分为3组(n=18):假手术(S)组、缺血再灌注(IR)组、丙泊酚(P)组.每组再根据再灌注时间(2、4、6h)的不同各分成3个亚组,分别为S 2、S 4、S 6、IR 2、IR 4、IR 6、P 2、P 4、P 6亚组.制备70%肝缺血再灌注模型,测定血液中胱抑素C(Cys-c),利用免疫组化方法检测各组肾组织中Bcl-2蛋白和Bax的表达,TUNEL法检测肾脏细胞的凋亡指数(AI).结果 各组同一时间点相比,IR组和P组的Cys-c、Bcl-2、Bax蛋白含量,AI值均显著高于S组(P<0.05).P组的Bcl-2、Bcl-2/Bax显著高于IR组,Bax、Cys-c、AI值显著低于IR组(P<0.05).结论 丙泊酚对肝缺血再灌注时肾脏损伤有保护作用,而其保护机制可能与调控Bcl-2、Bax蛋白的表达有关.

Objective To investigate the effect of post-conditioning with Propofol on apoptosis and the expression of bcl-2,bax gene in renal tissue after hepatic ischemia-reperfusion injury and its mechanism.Methods Fifty-four healthy male SD rats were randomly divided into three groups,including sham group (S),ischemia-reperfusion group (IR),propofol group (P).According to the different periods after reperfusion,each group was further divided into 2-hour 4-hour and 6-hour reperfusion subgroups respectively (n=6 in each subgroup),named S2、S4、S6 subgroups,andI R2、IR4、IR6 subgroups,and P2、P4、P6 subgroups.Make the 70% hepatic ischemia-reperfusion model.The blood was sampled at the end of ischemia-reperfusion for assay of cystatin c(Cys-c)the apoptosis rate(AI) in ischemia-reperfusion renal tissure were measured by TUNEL method; the bcl-2,bax concentration were measured by immuno-histochemistry.Results Different group compared to the sametime.Blood concentrations of Cys-c,AI,and the amount of bcl-2 and bax protein after reperfusion in group IR,and group P were higher than those in group S at matched reperfusion time points (P<0.05).The amount of bcl-2 and bcl-2/bax in group P were higher than those in group IR,but the amount of bax,AI and the concentrations of Cys-c in group P were lower than those in group IR (P<0.05).Conclusion Propofol can alleviate renal injury induced by hepatic ischemia-reperfusion in rats,in which the bcl-2 and bax protein may play important roles.
