目的：评价九味镇心颗粒合并安非他酮治疗首发抑郁症患者的临床疗效及安全性。方法将126例首发抑郁症患者随机分为研究组（62例，接受安非他酮合并九味镇心颗粒治疗）和对照组（64例，接受安非他酮治疗），于治疗前及治疗后第2、4、6、8周末分别采用汉密尔顿抑郁量表（ HAMD）、汉密尔顿焦虑量表（ HAMA）、治疗中需处理的不良反应症状量表（ TESS）进行疗效和不良反应评估。结果两组治疗后第2、4、6、8周末两组HAMD、HAMA总分均较各自治疗前下降（P＜0．01）；治疗后第6、8周末研究组HAMD总分低于对照组（P＜0．05，P＜0．01）；治疗后第2、4、6、8周末研究组HAMA评分低于对照组（P＜0．01）。研究组（62．00％）显效率高于对照组（58．38％）（P＜0．01）。两组总不良反应发生率比较差异无统计学意义（P＞0．05）。结论九味镇心颗粒合并安非他酮对首发抑郁症治疗可以更快缓解焦虑症状并提高抗抑郁疗效。
목적：평개구미진심과립합병안비타동치료수발억욱증환자적림상료효급안전성。방법장126례수발억욱증환자수궤분위연구조（62례，접수안비타동합병구미진심과립치료）화대조조（64례，접수안비타동치료），우치료전급치료후제2、4、6、8주말분별채용한밀이돈억욱량표（ HAMD）、한밀이돈초필량표（ HAMA）、치료중수처리적불량반응증상량표（ TESS）진행료효화불량반응평고。결과량조치료후제2、4、6、8주말량조HAMD、HAMA총분균교각자치료전하강（P＜0．01）；치료후제6、8주말연구조HAMD총분저우대조조（P＜0．05，P＜0．01）；치료후제2、4、6、8주말연구조HAMA평분저우대조조（P＜0．01）。연구조（62．00％）현효솔고우대조조（58．38％）（P＜0．01）。량조총불량반응발생솔비교차이무통계학의의（P＞0．05）。결론구미진심과립합병안비타동대수발억욱증치료가이경쾌완해초필증상병제고항억욱료효。
Objective To evaluate the clinical efficacy and safety of nine town heart particles combined with bupropion in treatmentoffirst-episodedepression.Methods 126patientswithfirst-episodedepressionwererandomlydividedintostudygroup (62 cases) treated with nine town heart particles combined with bupropion and control group (64 cases) treated with bupropion monotherapy for 8 weeks.All patients were assessed with Hamilton Depression Rating Scale ( HAMD) , Hamilton Anxiety Scale (HAMA) and Emergent Symptoms Scale (TESS) at baseline and at the end of the 2nd, 4th, 6th and 8th week of the treatment to evaluate the efficacy and adverse reaction.Results Total scores of HAMD and HAMA in both groups decreased significantly at the end of the 2nd, 4th, 6th and 8th week when compared with the baseline (P<0.01).Total score of HAMD in study group was significantly lower than that in control group at the end of the 6th (P<0.05) and 8th week (P<0.01).Total score of HAMA in study group was significantly lower than that in control group at the end of the 2nd, 4th, 6th and 8th week of the treatment (P <0.01).The response rate in study group was significantly higher than that in control group (62.00% vs. 58.53%, P<0.01).There was no significant difference in total frequency of adverse reactions between the two groups (P>0.05).Conclusion Nine town heart particles combined with bupropion can reduce anxiety symptoms quickly and enhance the antidepressant efficacy in the treatment of first-episode depression.
