目的:研究颈椎前路显微减压联合颈椎 Zero-P 植入治疗颈椎病的临床疗效。方法2011年4月至2013年12月,对41例颈椎病患者共52个椎间隙行前路显微镜下椎间盘减压联合颈椎 Zero-P 系统植入术,其中男性25例,女性16例;年龄36~71岁,平均年龄53.1岁。诊断为脊髓型25例,神经根型9例,混合型7例。随访时进行视觉模拟评分(visual analogue scale,VAS)、日本骨科协会(Japanese orthopaedic association,JOA)评分和颈椎残障功能指数(neck disability index,NDI),并采用配对 t 检验对不同时间点的评分进行分析。结果患者术后随访6~36个月,平均25.4个月。患者术后随访期间症状均较术前明显缓解,术后随访各时间点与术前比较,VAS 评分、JOA 评分和NDI 评分差异均有统计学意义(P ﹤0.05);术后各随访时间点,症状有进一步改善趋势,但差异均无统计学意义(P ﹥0.01)。结论颈椎前路 Zero-P 系统植入联合显微减压手术能彻底去除对神经组织产生压迫的致压物,疗效更有保障。

목적:연구경추전로현미감압연합경추 Zero-P 식입치료경추병적림상료효。방법2011년4월지2013년12월,대41례경추병환자공52개추간극행전로현미경하추간반감압연합경추 Zero-P 계통식입술,기중남성25례,녀성16례;년령36~71세,평균년령53.1세。진단위척수형25례,신경근형9례,혼합형7례。수방시진행시각모의평분(visual analogue scale,VAS)、일본골과협회(Japanese orthopaedic association,JOA)평분화경추잔장공능지수(neck disability index,NDI),병채용배대 t 검험대불동시간점적평분진행분석。결과환자술후수방6~36개월,평균25.4개월。환자술후수방기간증상균교술전명현완해,술후수방각시간점여술전비교,VAS 평분、JOA 평분화NDI 평분차이균유통계학의의(P ﹤0.05);술후각수방시간점,증상유진일보개선추세,단차이균무통계학의의(P ﹥0.01)。결론경추전로 Zero-P 계통식입연합현미감압수술능철저거제대신경조직산생압박적치압물,료효경유보장。
Objective To study the clinical effect of microsurgical decompression combined with cervical Zero-P implan-ted for the treatment of cervical spondylosis. Methods From April 2011to December 2013,41 cases of cervical spondylosis, including 52 intervertebral spaces,underwent microscope disc decompression and cervical Zero-P implanted. There were 25 male and 16 female patients;aged from 36 to 71years,with an average of 53. 1 years. There were 25 cases of cervical spondylot-ic myelopathy,9 cases of nerve root type cervical spondylosis and 7 cases of mixedcervical spondylosis. Visual analogue scale (VAS)scores,Japanese 0rthopedic Association(JOA)scores and neck disability index(NDI)scores were recorded and com-pared with preoperative scores by the paired t-test. Results The patients were followed up for 6 to 36 months,with an average of 25. 4 months. During follow up,the patients symptoms were obvious alleviated and had statistically significant improvement as assessed by the JOA scores,NDI scores and VAS scores compared pre-opration(P ﹤ 0. 05). The symptoms of the patients shown tendency to improve at follow-up point post-opration,but there were no statistically significant relationship between each point(P ﹥ 0. 01). Conclusion The cervical Zero-P system implantation combined with microscopic decompression surgery can be effective decompression of nerve tissue.
