目的:观察两种不同的取材及检测方法,对乳腺癌乳头乳晕复合体浸润检出率的影响。方法460例确诊为 I、II 期乳腺癌的病理标本,随机分为两组,每组230例,实验组按乳头乳晕复合体全面取材;对照组从乳头正中矢状面取材。标本连续切片,分别行 HE 染色及 CK-19免疫组化染色,了解乳头乳晕复合体乳腺癌浸润情况。结果实验组 HES 阳性14例(6.09%),对照组 HES 阳性4例(1.74%,χ2=5.782, P<0.05);实验组 IHC 阳性27例(11.74%),对照组 IHC阳性11例(4.78%,χ2=7.343, P<0.01)。 IHC 与 HES 相比,在实验组中差别有显著性意义(χ2=4.526, P<0.05),但在对照组差别无显著性意义(χ2=3.377, P>0.05)。结论乳头乳晕复合体全面取材有更高的 NAC 乳腺癌浸润的检出率;CK-19免疫组化染色比传统 HE 染色更能发现 NAC 的微浸润灶。

목적:관찰량충불동적취재급검측방법,대유선암유두유훈복합체침윤검출솔적영향。방법460례학진위 I、II 기유선암적병리표본,수궤분위량조,매조230례,실험조안유두유훈복합체전면취재;대조조종유두정중시상면취재。표본련속절편,분별행 HE 염색급 CK-19면역조화염색,료해유두유훈복합체유선암침윤정황。결과실험조 HES 양성14례(6.09%),대조조 HES 양성4례(1.74%,χ2=5.782, P<0.05);실험조 IHC 양성27례(11.74%),대조조 IHC양성11례(4.78%,χ2=7.343, P<0.01)。 IHC 여 HES 상비,재실험조중차별유현저성의의(χ2=4.526, P<0.05),단재대조조차별무현저성의의(χ2=3.377, P>0.05)。결론유두유훈복합체전면취재유경고적 NAC 유선암침윤적검출솔;CK-19면역조화염색비전통 HE 염색경능발현 NAC 적미침윤조。
Objective To observe the impact of different sampling and testing methods on the detection rate of nipple—areolar complex (NAC) in breast cancer.Methods 460 samples with the breast cancer on stage I or II were randomly divided into the two groups, with whole-transection in the experimental group (n=230) and midline-transection in the control group (n=230).The sam-ples were cut into slices continuously and were detected with HE staining (HES) and cytokeratin-19 (CK-19) monoclonal antibody immunohistochemical examination (IHC).Results There were 14 cases (6.09 %) of positive HES in the experimental group, and 4 cases (1.74 %) in the control group (χ2 =5.782, P<0.05), there were 27 cases (11.74 %) of positive IHC in the experimental group, and 11 cases (4.78 %) in the control group (χ2 =7.734 P<0.01).IHC had obvious discrepancy compared with HES in the experimental group (χ2 =4.526, P<0.05), but had no discrepancy in the control group (χ2 =3.377, P>0.05).Conclusion NAC whole-transection is more comprehensive in identifying NAC involvement than midline-transection and CK-19 IHC is more sensi-tive in detecting NAC micro-involvement than HES.
