目的：探讨经尿道膀胱肿瘤电切术（ TURBT）后吉西他滨（ GEM）和吡柔比星（ THP）交替膀胱灌注化疗预防膀胱癌术后复发的疗效。方法92例膀胱尿路上皮癌（ BUC）患者TURBT术后经尿道插管定期交替膀胱灌注GEM和 THP，总时限19个月。分别在术后第3、6、12个月时用膀胱镜或CT复查随访。治疗结束后，根据前次检查情况每6个月或12个月时继续复查，连续5年。结果5年期间局部复发23例，复发率为26．4％（23／87），局部复发＋转移4例，复发转移率4．6％（4／87），死亡2例，死亡率2．3％（2／87），死于与本病不相关9例，失访5例。结论 GEM和THP作为表浅性膀胱癌TURBT术后辅助化疗药物，对于改善预后、减少复发转移和降低死亡率疗效确实，且副作用小，可能成为提高术后疗效的重要手段之一。
목적：탐토경뇨도방광종류전절술（ TURBT）후길서타빈（ GEM）화필유비성（ THP）교체방광관주화료예방방광암술후복발적료효。방법92례방광뇨로상피암（ BUC）환자TURBT술후경뇨도삽관정기교체방광관주GEM화 THP，총시한19개월。분별재술후제3、6、12개월시용방광경혹CT복사수방。치료결속후，근거전차검사정황매6개월혹12개월시계속복사，련속5년。결과5년기간국부복발23례，복발솔위26．4％（23／87），국부복발＋전이4례，복발전이솔4．6％（4／87），사망2례，사망솔2．3％（2／87），사우여본병불상관9례，실방5례。결론 GEM화THP작위표천성방광암TURBT술후보조화료약물，대우개선예후、감소복발전이화강저사망솔료효학실，차부작용소，가능성위제고술후료효적중요수단지일。
Objective To observe the efficiency of relapse prevention of alternating bladder perfusion with GEM and THP after tran -surethralresectionofbladdertumor(TURBT)ofsuperficilbladdercancer.Methods 92bladderurothelialcarcinoma(BUC)patientsafter TURBT were given alternating bladder perfusion with GEM and THP .After treatment for 3,6 and 12 months,the patients were followed up by cystoscope or CT.After the end of treatment ,the review was carried out every 6 or 12 months according to the previous results for 5 consecu-tive years.Results During 5 years,23 cases(26.4%) was local recurrence,4 cases(4.6%) of local recurrence and metastasis,2 cases (2.3%) of death.9 cases died of the other disease ,5 cases lost follow-up.Conclusions GEM and THP as the adjuvant chemotherapy drugs after TURBT of superficial bladder cancer patients ,could improve curative effects ,decrease the recurrence and mortality rates , which might become one of the important means to improve the postoperative curative effect .
