目的：探讨通心络胶囊对早期糖尿病肾病（DN）患者血管内皮功能及肾功能的影响。方法选择2012年4月—2013年6月住院的DN患者100例，按照随机数字表法分为对照组和观察组各50例，2组患者均注射胰岛素降糖、他汀类降脂药及硫酸氢氯吡格雷等常规基础药物治疗，观察组在常规治疗基础上加服通心络胶囊。2组均治疗12周。观察治疗前后2组患者血管内皮功能及肾功能的变化。结果治疗前2组患者血管内皮功能及肾功能比较差异无统计学意义（ P ＞0．05）。与治疗前比较，治疗后2组患者血管内皮生长因子（VEGF）、一氧化氮（NO）均升高（ t 对＝1．878、1．934；t 观＝1．971、1．952， P均＜0．05），内皮素-1（ET-1）、血肌酐（SCr）、尿素氮（BUN）、肌酐清除率（CCr）、胱抑素-C（Cys-C）、肿瘤坏死因子-α（TNF-α）均降低（ t 对＝1．846、1．792、1．813、1．885、1．861、1．916；t观＝1．891、1．808、1．900、2．034、2．012、1．978， P均＜0．05）。与对照组治疗后比较，观察组VEGF、NO、均明显升高（ t ＝2．265、3．348， P均＜0．05），ET-1、SCr、BUN、CCr、Cys-C、TNF-α均明显降低（ t ＝2．461、2．267、2．166、2．413、2．201、2．332， P均＜0．05）。结论通心络胶囊能明显改善DN患者肾功能、血管内皮功能，降低体内炎性因子水平。
목적：탐토통심락효낭대조기당뇨병신병（DN）환자혈관내피공능급신공능적영향。방법선택2012년4월—2013년6월주원적DN환자100례，안조수궤수자표법분위대조조화관찰조각50례，2조환자균주사이도소강당、타정류강지약급류산경록필격뢰등상규기출약물치료，관찰조재상규치료기출상가복통심락효낭。2조균치료12주。관찰치료전후2조환자혈관내피공능급신공능적변화。결과치료전2조환자혈관내피공능급신공능비교차이무통계학의의（ P ＞0．05）。여치료전비교，치료후2조환자혈관내피생장인자（VEGF）、일양화담（NO）균승고（ t 대＝1．878、1．934；t 관＝1．971、1．952， P균＜0．05），내피소-1（ET-1）、혈기항（SCr）、뇨소담（BUN）、기항청제솔（CCr）、광억소-C（Cys-C）、종류배사인자-α（TNF-α）균강저（ t 대＝1．846、1．792、1．813、1．885、1．861、1．916；t관＝1．891、1．808、1．900、2．034、2．012、1．978， P균＜0．05）。여대조조치료후비교，관찰조VEGF、NO、균명현승고（ t ＝2．265、3．348， P균＜0．05），ET-1、SCr、BUN、CCr、Cys-C、TNF-α균명현강저（ t ＝2．461、2．267、2．166、2．413、2．201、2．332， P균＜0．05）。결론통심락효낭능명현개선DN환자신공능、혈관내피공능，강저체내염성인자수평。
Objective To investigate the effect of Tongxinluo capsule on vascular endothelial function and renal func-tion in patients with early diabetic nephropathy (DN).Methods From April 2012 to June 2013, 100 cases of DN patients were divided into control group and observation group , 50 cases in each group , according to random number table , 2 groups of patients were given insulin hypoglycemic , statin and Clopidogrel Hydrogen Sulfate and other conventional treatment , observa-tion group on the basis of routine treatment , also added Tongxinluo capsule.Two groups were treated for 12 weeks.The chan-ges of vascular endothelial function and renal function were observed before and after treatment .Results Before treatment , 2 group patients ’ vascular endothelial function and renal function ’ s differences did not show statistical significance ( P >0.05).After treatment, two groups of patients ’ vascular endothelial growth factor ( VEGF), nitric oxide ( NO) were in-creased (control group:t =1.878,1.934,observation group:t =1.971,1.952, P <0.05), endothelin 1 (ET-1), serum creatinine (SCr) and blood urea nitrogen (BUN), creatinine clearance rate (CCR) and cystatin C (Cys C), tumor necrosis factor alpha (TNF-α) were decreased (control group:t =1.846,1.792,1.813,1.885,1.861,1.916,observation group:t =1.891,1.808,1.900,2.034,2.012,1.978, P <0.05).Compared with the control group, observation group’s vascular en-dothelial growth factor (VEGF), nitric oxide (NO) were significantly increased ( t =2.265,3.348, P <0.05), and ET-1, SCr, BUN, CCR, Cys C, TNF alpha were significantly reduced ( t =2.461,2.267,2.166,2.413,2.201,2.332, P <0 .05).Conclusion It demonstrated that the Tongxinluo capsule could obviously improve the DN patients ’ renal function, vascular endothelial function , reduce inflammatory factor levels .
