目的:探讨高频振荡通气治疗新生儿呼吸衰竭并气胸的疗效。方法33例呼吸衰竭并气胸的新生儿常频机械通气(CMV)失败后改为高频振荡通气(HFOV)治疗,治疗过程中连续监测呼吸机参数[吸入氧浓度(FiO2)、平均气道压(MAP)、压力振荡幅度(△P)]、经皮氧饱和度、心率、血压、动脉血气分析等。结果33例患儿用HFOV治疗后,1例因经济原因高频振荡通气36 h后放弃治疗,余32例高频振荡通气治疗48 h,吸入氧浓度、平均气道压、振幅均明显降低,氧分压明显升高,差异有统计学意义(P<0.05),治疗期间心率、血压无明显变化。结论高频振荡通气治疗新生儿呼吸衰竭并气胸疗效显著,优于常频机械通气,值得推广。

목적:탐토고빈진탕통기치료신생인호흡쇠갈병기흉적료효。방법33례호흡쇠갈병기흉적신생인상빈궤계통기(CMV)실패후개위고빈진탕통기(HFOV)치료,치료과정중련속감측호흡궤삼수[흡입양농도(FiO2)、평균기도압(MAP)、압력진탕폭도(△P)]、경피양포화도、심솔、혈압、동맥혈기분석등。결과33례환인용HFOV치료후,1례인경제원인고빈진탕통기36 h후방기치료,여32례고빈진탕통기치료48 h,흡입양농도、평균기도압、진폭균명현강저,양분압명현승고,차이유통계학의의(P<0.05),치료기간심솔、혈압무명현변화。결론고빈진탕통기치료신생인호흡쇠갈병기흉료효현저,우우상빈궤계통기,치득추엄。
Objective To investigate curative effect by high frequency oscillatory ventilation in the treatment of neonatal respiratory failure complicated with pneumothorax. Methods There were 33 newborns of neonatal respiratory failure complicated with pneumothorax receiving high frequency oscillatory ventilation (HFOV) after failed conventional mechanical ventilation (CMV). Continuous monitoring was made on respirator indexes[fraction of inspired oxygen (FiO2), mean airway pressure (MAP), pressure oscillation amplitude (△P)], transcutaneous oxygen saturation, heart rate, blood pressure, and arterial blood gas analysis. Results After receiving HFOV for treatment, there was 1 case among the 33 cases gave up the treatment after 36 h due to economical reason. The other 32 cases received HFOV for 48 h. They had obvious reduced fraction of inspired oxygen, mean airway pressure, oscillation amplitude and increased oxygen partial pressure. The difference had statistical significance (P<0.05). There were no obvious changes of heart rate and blood pressure during treatment. Conclusion High frequency oscillatory ventilation provides better effect than conventional mechanical ventilation in treating neonatal respiratory failure complicated with pneumothorax, and this method is worth promoting.
