目的：探讨不同程度的糖尿病合并肺结核患者的肺部病变基质金属蛋白酶-9（MMP-9）、组织金属蛋白酶抑制剂-1（TIMP-1）表达水平的差异。方法：选取本院住院的肺结核合并糖尿病患者肺组织60例（A组），肺结核非糖尿病患者肺组织47例（B组）及正常肺组织45例（C组），采用免疫组化方法检测肺组织中MMP-9、TIMP-1表达水平，并收集患者肺结核相关临床资料进行统计分析。结果：A组的MMP-9、TIMP-1、MMP-9/TIMP-1显著高于B、C组，差异均有统计学意义（P<0.05）。B组的MMP-9、TIMP-1及MMP-9/TIMP-1较C组高，差异均有统计学意义（P<0.05）。活动性肺结核患者MMP-9、MMP-9/TIMP-1高于非活动性肺结核患者，活动性肺结核患者TIMP-1表达低于非活动性肺结核患者（痰抗酸杆菌阴性，X片显示增殖性病变、纤维包囊紧密的干酪硬结灶及纤维钙化灶）。结论：MMP-9、TIMP-1及其比值与肺结核患者的病变程度密切相关，MMP-9/TIMP-1表达失衡在肺结核合并糖尿病的发病过程中发挥着重要作用。
목적：탐토불동정도적당뇨병합병폐결핵환자적폐부병변기질금속단백매-9（MMP-9）、조직금속단백매억제제-1（TIMP-1）표체수평적차이。방법：선취본원주원적폐결핵합병당뇨병환자폐조직60례（A조），폐결핵비당뇨병환자폐조직47례（B조）급정상폐조직45례（C조），채용면역조화방법검측폐조직중MMP-9、TIMP-1표체수평，병수집환자폐결핵상관림상자료진행통계분석。결과：A조적MMP-9、TIMP-1、MMP-9/TIMP-1현저고우B、C조，차이균유통계학의의（P<0.05）。B조적MMP-9、TIMP-1급MMP-9/TIMP-1교C조고，차이균유통계학의의（P<0.05）。활동성폐결핵환자MMP-9、MMP-9/TIMP-1고우비활동성폐결핵환자，활동성폐결핵환자TIMP-1표체저우비활동성폐결핵환자（담항산간균음성，X편현시증식성병변、섬유포낭긴밀적간락경결조급섬유개화조）。결론：MMP-9、TIMP-1급기비치여폐결핵환자적병변정도밀절상관，MMP-9/TIMP-1표체실형재폐결핵합병당뇨병적발병과정중발휘착중요작용。
Objective:To compare the expression level of MMP-9 and TIMP-1 and explore it’s significance in patients of tuberculosis combined with diabetes.Method:The patients in our hospital were selected and divided into three groups:group A of 60 patients with pulmonary tuberculosis and diabetes,group B of 47 patients with benign lung lesions,group C of 45 patients without tuberculosis. Immunocytochemistry method was used to test the expression level of MMP-9 and TIMP-1 in the lung lesions. The clinical data of patients with pulmonary tuberculosis were collected and analyzed statistically.Result:The expression level of MMP-9,TIMP-1 and MMP-9/TIMP-1 had significance in three groups,the expression level of group A was the highest,the next was group B,the differences were statistically significant(P<0.05).The expression levels of MMP-9,MMP-9/TIMP-1 in the patients with active tuberculosis were higher than the patients with non-active tuberculosis ,however the expression level of TIMP-1 in patients with active tuberculosis was less than patients with no-active tuberculosis(sputum negative which acid fast bacilli,X-ray showed hyperplastic lesions,fiber cyst closely cheese scleroma focal calcifications).Conclusion:The expression level of MMP-9 and TIMP-1 and its ratio are closely related to the degree of the pathological changes of pulmonary tuberculosis patients,imbalances of expression of MMP-9/TIMP-1 in pulmonary tuberculosis plays an important role in the process of the onset of diabetes.
