目的：探讨可溶性尿激酶型纤溶酶原激活物受体（suPAR）在成人社区获得性肺炎（CAP）住院患者中的表达及临床意义。方法：采用用酶联免疫吸附试验（ELISA）法测定61例住院CAP患者治疗前后及60例健康对照者的血浆suPAR水平，测定所有研究对象的CRP、WBC及PCT水平，CAP患者入院时进行肺炎CURB-65及APACHEⅡ评分，分析血浆suPAR与PCT、CRP、CURB-65、APACHEⅡ及住院时间的相关性。结果：肺炎治疗前suPAR、CRP、WBC及PCT水平均显著高于对照组，肺炎治疗后suPAR、CRP、WBC及PCT水平较治疗前显著降低，以上各项比较差异均有统计学意义（P<0.01）；重症肺炎组住院时间、APACHEⅡ评分、CURB-65评分及血suPAR、CRP、WBC、PCT含量均显著高于非重症肺炎组，比较差异均有统计学意义（P<0.01）；血浆suPAR水平与PCT、CRP水平，CURB-65、APACHEⅡ评分及住院时间有相关性。结论：suPAR对社区获得性肺炎有一定的诊断价值， suPAR水平对肺炎的病情程度有一定的评估作用，在指导治疗等方面具有一定的价值。
목적：탐토가용성뇨격매형섬용매원격활물수체（suPAR）재성인사구획득성폐염（CAP）주원환자중적표체급림상의의。방법：채용용매련면역흡부시험（ELISA）법측정61례주원CAP환자치료전후급60례건강대조자적혈장suPAR수평，측정소유연구대상적CRP、WBC급PCT수평，CAP환자입원시진행폐염CURB-65급APACHEⅡ평분，분석혈장suPAR여PCT、CRP、CURB-65、APACHEⅡ급주원시간적상관성。결과：폐염치료전suPAR、CRP、WBC급PCT수평균현저고우대조조，폐염치료후suPAR、CRP、WBC급PCT수평교치료전현저강저，이상각항비교차이균유통계학의의（P<0.01）；중증폐염조주원시간、APACHEⅡ평분、CURB-65평분급혈suPAR、CRP、WBC、PCT함량균현저고우비중증폐염조，비교차이균유통계학의의（P<0.01）；혈장suPAR수평여PCT、CRP수평，CURB-65、APACHEⅡ평분급주원시간유상관성。결론：suPAR대사구획득성폐염유일정적진단개치， suPAR수평대폐염적병정정도유일정적평고작용，재지도치료등방면구유일정적개치。
Objective:To investigate the expression and clinical significance of soluble urokinase-type plasminogen activator receptor (suPAR) in patients with CAP.Method:The plasma suPAR levels of 61 adult patients with CAP and 60 healthy patients were measured by the method of enzyme-linked immunosorbent assay(ELISA).The levels of CRP、WBC and PCT of all research objects were measured.The APACHE Ⅱ and CURB-65 scores were evaluated when the patients were hospitalized. The relationship between plasma suPAR and PCT,CRP,CURB-65 score,APACHEⅡ score,length of stay were analyzed. Result:Before treatment,the suPAR,CRP、WBC and PCT levels in patients with CAP were significantly higher than those in the control group,after treatment,the suPAR,CRP、WBC and PCT levels of CAP patients were lower than before treatment, the differences above were all statistically significant(P<0.01).The hospital stays,CURB-65 score,APACHEⅡ score and the suPAR,CRP,WBC,PCT levels in the SCAP patients were much higher than those in the NSCAP patients,the differences were all statistically significant(P<0.01).The plasma levels of suPAR was correlated with the CURB-65 score,APACHEⅡ score,length of hospital stay and levels of CRP,PCT.Conclusion:suPAR may play a role in the diagnosis and clinical assessment of the severity of CAP,it has a certain value in guiding therapeutic.
