目的:观察重组人粒细胞集落刺激因子( rhG?CSF)混合液治疗恶性肿瘤化疗引起口腔溃疡的临床效果。方法将50例恶性肿瘤患者按数字表法随机分为观察组和对照组,每组25例。对照组采用0?9%氯化钠注射液100 ml加冰硼散3 g、锡类散1 g配制的含漱液。观察组为在对照组基础上加入rhG?CSF 100μg、庆大霉素16万U、地塞米松5 mg及2%利多卡因10 ml配制的含漱液。两组患者均予10 ml/次含漱,每日3~5次,每次3 min;从发生口腔溃疡24 h内开始用药,至口腔溃疡愈合或治疗7d后停药。观察并记录两组患者口腔溃疡的治疗有效率、疼痛缓解率、疼痛缓解时间及溃疡愈合时间。结果观察组口腔溃疡的治疗有效率为92?0%,高于对照组的60?0%( P<0?05);观察组的疼痛缓解率优于对照组(84?0% vs.52?0%),差异有统计学意义( P<0?05);给药后观察组不同时间点的口腔溃疡疼痛评分均显著低于对照组( P<0?05);观察组的口腔溃疡平均愈合时间明显短于对照组[(3?5±1?2) d vs.(5?7±1?3) d,P<0?05]。结论 rhG?CSF混合液含漱治疗化疗后引起的口腔溃疡效果显著,能有效减轻患者的疼痛程度和时间,加快创面愈合,明显缩短口腔溃疡的愈合时间。

목적:관찰중조인립세포집락자격인자( rhG?CSF)혼합액치료악성종류화료인기구강궤양적림상효과。방법장50례악성종류환자안수자표법수궤분위관찰조화대조조,매조25례。대조조채용0?9%록화납주사액100 ml가빙붕산3 g、석류산1 g배제적함수액。관찰조위재대조조기출상가입rhG?CSF 100μg、경대매소16만U、지새미송5 mg급2%리다잡인10 ml배제적함수액。량조환자균여10 ml/차함수,매일3~5차,매차3 min;종발생구강궤양24 h내개시용약,지구강궤양유합혹치료7d후정약。관찰병기록량조환자구강궤양적치료유효솔、동통완해솔、동통완해시간급궤양유합시간。결과관찰조구강궤양적치료유효솔위92?0%,고우대조조적60?0%( P<0?05);관찰조적동통완해솔우우대조조(84?0% vs.52?0%),차이유통계학의의( P<0?05);급약후관찰조불동시간점적구강궤양동통평분균현저저우대조조( P<0?05);관찰조적구강궤양평균유합시간명현단우대조조[(3?5±1?2) d vs.(5?7±1?3) d,P<0?05]。결론 rhG?CSF혼합액함수치료화료후인기적구강궤양효과현저,능유효감경환자적동통정도화시간,가쾌창면유합,명현축단구강궤양적유합시간。
Objective To investigate the clinical efficacy of recombinant human granulocyte colony?stimulating factor ( rhG?CSF) collutory on chemotherapy?induced oral ulcer. Methods Fifty cases of malignant tumors patients were divided into control group and observation group with 25 cases in each group according to random number table.The control group was treated by Bingpeng pow?der,Xilei powder plus 100 ml normal saline with gargling administration(10 ml each time, 3?5 times daily, 3 min for each gargling) and the observation group was treated by mouthwash containing 100 ml normal saline, rhG?CSF 100 μg, dexa?methasone 5 mg, lido?caine 10 ml,gentamicin 160 thousand unit, Bingpeng powder and Xilei powder with the same usage and dosage of control group. Two groups were given mouthwash from the occurrence of oral ulcer within 24 hours until oral ulcer healing or completing 7 days of mouth rinse. The overall response rate of oral ulcer healing, remission rate of pain, the time of alleviating pain and the average healing time of oral ulcer were observed and recorded. Results In observation group, the response rate of oral ulcer healing was 92?0%, while in control group it was only 60?0% with statistic significance( P<0?05) . The pain remission rate of observation group was 84?0%,which was significantly superior to that of the control group(52?0%) with significant difference(P<0?05). After treatment, the pain scores (VAS) of observation group improved more than that of control group at different time points(P<0?05).The average healing time of o?ral ulcer in observation group was significantly shorter than that of control group[(3?5±1?2) d vs.(5?7±1?3) d,P<0?05]. Conclu?sion The efficacy of rhG?CSF collutory on treating chemotherapy?induced oral ulcer is satisfactory. It can effectively reduce the severi?ties of oral pain and the time of alleviating pain,improve oral ulcer healing,and obviously shorten the healing time of oral ulcer.
