目的:探讨不同分期DR患者血清可溶性CD40配体(SCD40L )水平的变化及其临床意义。方法将91例T2DM患者分为单纯T2DM组36例、非增殖型DR(NPDR)组30例和增殖型DR(PDR)组25例,另选体检健康者25名为健康对照(NC)组。采用ELISA检测各组血清SCD40L水平。结果T2DM组血清SCD40L水平较NC组高[(310.17±37.78) vs (200.34±14.21)pg/ml ,P<0.05],NPDR、PDR组较T2DM 组高[(384.89±37.78),(480.52±47.17) vs (310.17±37.03)pg/ml ,P<0.05],且PDR组较NPDR组高[(480.52±47.17) vs (384.89±37.78)pg/ml ,P<0.05]。Logistic回归分析结果显示,病程、SCD40L为DR的影响因素。结论 DR患者血清SCD40L水平升高,SCD40L在DR发生发展中起重要作用。

Objective To investigate the change and clinical significance of serum soluble CD40 ligand (SCD40L) level in different stages of DR. Methods 91 T2DM patient were divided into three groups :T2DM without DR (T2DM ) group (n=36) ,T2DM with nonproliferative DR (NPDR) group (n=30) and T2DM with proliferative DR (PDR) group (n=25).Healthy subjects in routine examination were selected as normal control (NC) group (n=25). Serum SCD40L levels were measured by ELISA in each group. Results Serum SCD40L level was significantly higher in T2DM group than in NC group [(310.17 ± 37.78) vs (200.34 ± 14.21)pg/ml ,P<0.05]. Serum SCD40L level was higher in NPDR and PDR groups than in T2DM group [(384.89 ± 37.78) vs (310.17 ± 37.03)pg/ml ,P< 0.05;(480.52 ± 47.17) vs (310.17 ± 37.03)pg/ml ,P< 0.05]. Serum SCD40L level was higher in PDR group than in NPDR group [(480.52 ± 47.17) vs (384.89 ± 37.78)pg/ml ,P<0.05]. Logistic multivariate regression analysis showed that ,T2DM duration and SCD40L were associated factors for DR. Conclusion Serum SCD40L level increased in DR patients. Serum SCD40L may play an important role in the progress of DR.
