目的:观察肥胖T2DM患者血清铁蛋白(SF)水平与IR的关系。方法选取 T2DM 患者100例,根据BMI分为糖尿病合并肥胖组(DM+Ob ,BMI≥28 kg/m2)54例和单纯糖尿病(DM ,BMI<28 kg/m2)组46例。另选取同期正常体检者50名作为对照(NC)组。检测各组SF、FPG、FIns、TC、TG及HbA1c水平,并计算胰岛素抵抗指数(HOMA‐IR)。观察SF水平和HOMA‐IR的变化,分析两者的相关性。结果 DM+ Ob组SF[(292.66±88.53) vs (167.52±67.04) vs (152.17±50.12) ng/ml]及HOMA‐IR[(4.20±1.61) vs (1.87±0.97),(1.73±0.86)]均高于DM组和NC组(P<0.05)。Pearson相关分析显示,SF与 HOMA‐IR呈正相关(r=0.79,P<0.05)。多元逐步回归分析显示,SF与 SBP、HbA1c、BMI、HOMA‐IR呈正相关(P<0.05)。结论 T2DM 肥胖患者SF、HOMA‐IR水平高于单纯T2DM患者,SF与 HOM A‐IR呈正相关。

목적:관찰비반T2DM환자혈청철단백(SF)수평여IR적관계。방법선취 T2DM 환자100례,근거BMI분위당뇨병합병비반조(DM+Ob ,BMI≥28 kg/m2)54례화단순당뇨병(DM ,BMI<28 kg/m2)조46례。령선취동기정상체검자50명작위대조(NC)조。검측각조SF、FPG、FIns、TC、TG급HbA1c수평,병계산이도소저항지수(HOMA‐IR)。관찰SF수평화HOMA‐IR적변화,분석량자적상관성。결과 DM+ Ob조SF[(292.66±88.53) vs (167.52±67.04) vs (152.17±50.12) ng/ml]급HOMA‐IR[(4.20±1.61) vs (1.87±0.97),(1.73±0.86)]균고우DM조화NC조(P<0.05)。Pearson상관분석현시,SF여 HOMA‐IR정정상관(r=0.79,P<0.05)。다원축보회귀분석현시,SF여 SBP、HbA1c、BMI、HOMA‐IR정정상관(P<0.05)。결론 T2DM 비반환자SF、HOMA‐IR수평고우단순T2DM환자,SF여 HOM A‐IR정정상관。
Objective To investigate the relationship between the levels of serum ferritin (SF) and insulin resistance in obese patients with type 2 diabetes (T2DM ). Methods 100 T2DM patients were divided into two groups based on BMI:T2DM with obesity (DM+Ob group ,BMI≥28 kg/m2 ,n=54) and T2DM without obesity (DM group ,BMI<28 kg/m2 ,n=46) ,50 cases of healthy people were selected as normal control (NC group). The levels of SF ,FPG ,FIns ,TC ,TG ,and HbA1c were measured. Insulin resistance index (HOMA‐IR) was calculated by the formula. Results Both SF [(292.66 ± 88.53) vs (167.52 ± 67.04) vs (152.17 ± 50.12) ng/ml] and HOMA‐IR [(4.20 ± 1.61) vs (1.87 ± 0.97) ,(1.73 ± 0.86)] in DM + Ob group were higher than in DM and NC group (all P< 0.05).Pearson correlation analysis showed that SF was positively correlated with HOMA‐IR (r = 0.79 ,P< 0.05). Multiple regression analysis suggested that SF was correlated with SBP ,HbA1c ,BMI and HOMA‐IR(P<0.05).Conclusion The levels of SF and HOMA‐IR in obese type 2 diabetic patients is higher. There is a linear relationship between SF and HOMA‐IR.
