目的:探讨老龄对糖耐量正常人群血糖和胰岛β细胞功能的影响。方法采用横断面多中心整群随机抽取新疆地区居民2117名,采用胰岛β细胞功能指数(HOM A‐β)、早期相胰岛素分泌(AUCins/glu30 min )、总胰岛素分泌(AUCins/glu120 min )、早期相胰岛素处置指数(DI30 min )及总胰岛素处置指数(DI120 min )评估胰岛细胞功能,及胰岛素抵抗指数(HOMA‐IR)评估IS。结果(1)随年龄增长,FPG(P=0.004)和2 hPG(P<0.001)呈上升趋势,HOMA‐β(P=0.039)、AUCins/glu120 min (P=0.022)、DI120 min (P=0.023)、DI30 min (P<0.001)及AUCins/glu30 min (P=0.006)均呈降低趋势。(2)Logistic多元回归分析显示,胰岛β细胞功能与低龄、高BM I水平、良好控制饮食、受教育程度高及少饮酒相关。结论随年龄增长,胰岛β细胞功能及对IR的代偿能力逐渐减弱,血糖水平轻度升高,生活方式可能对年龄相关的胰岛β细胞功能下降有影响。

목적:탐토노령대당내량정상인군혈당화이도β세포공능적영향。방법채용횡단면다중심정군수궤추취신강지구거민2117명,채용이도β세포공능지수(HOM A‐β)、조기상이도소분비(AUCins/glu30 min )、총이도소분비(AUCins/glu120 min )、조기상이도소처치지수(DI30 min )급총이도소처치지수(DI120 min )평고이도세포공능,급이도소저항지수(HOMA‐IR)평고IS。결과(1)수년령증장,FPG(P=0.004)화2 hPG(P<0.001)정상승추세,HOMA‐β(P=0.039)、AUCins/glu120 min (P=0.022)、DI120 min (P=0.023)、DI30 min (P<0.001)급AUCins/glu30 min (P=0.006)균정강저추세。(2)Logistic다원회귀분석현시,이도β세포공능여저령、고BM I수평、량호공제음식、수교육정도고급소음주상관。결론수년령증장,이도β세포공능급대IR적대상능력축점감약,혈당수평경도승고,생활방식가능대년령상관적이도β세포공능하강유영향。
Objective To explore the influence of aging on βcell function and its related factors in normal glucose tolerance subjects. Methods 2117 inhabitants were investigated in a cross sectional survey by cluster random sampling method. HOMA‐β,AUCins/glu30 min ,AUCins/glu120 min ,DI30 min and DI120 min were used to evaluate beta cell function. HOMA‐IR was used to evaluate insulin sensitivity. Results (1) With the increase of age ,FPG(P=0.004) and 2 hPG (P<0.001) increased ,HOMA‐β(P=0.039) , AUCins/glu120 min (P= 0.022 ) ,DI120 min ( P= 0.023 ) ,DI30 min ( P< 0.001 ) and AUCins/glu30 min ( P= 0.006 ) decreased. (2 ) Multiple logistic regression analysis showed that aging ,BMI ,lifestyle intervention , education levels and alcohol intake were related toβcell function. Conclusion With aging ,islet beta cell function and its compensation to insulin resistance decreased gradually ,glucose levels increased mildly.Lifestyle might have influence on the decrease of beta cell function.
