目的：探讨和研究基底动脉尖综合征(TOBS)临床特征及预后影响因素。方法对52例基底动脉尖综合征患者进行回顾性分析研究，分析其临床特征、预后影响因素。结果基底动脉尖综合征患者以意识障碍及头痛头晕为主要临床症状，以眼球运动障碍、瞳孔异常及肢体瘫痪为主要临床体征。糖尿病、高血压、慕下、慕上合并幕下、病灶个数>4个及格拉斯哥昏迷评分(GCS)≥10分是基底动脉尖综合征不良预后因素。结论通过患者临床症状及体征、影像学表现有助于对基底动脉尖综合征做出早期诊断和治疗，根据患者不良预后因素及早判断病情严重程度，进而早期全面治疗，可提高治愈率。
목적：탐토화연구기저동맥첨종합정(TOBS)림상특정급예후영향인소。방법대52례기저동맥첨종합정환자진행회고성분석연구，분석기림상특정、예후영향인소。결과기저동맥첨종합정환자이의식장애급두통두훈위주요림상증상，이안구운동장애、동공이상급지체탄탄위주요림상체정。당뇨병、고혈압、모하、모상합병막하、병조개수>4개급격랍사가혼미평분(GCS)≥10분시기저동맥첨종합정불량예후인소。결론통과환자림상증상급체정、영상학표현유조우대기저동맥첨종합정주출조기진단화치료，근거환자불량예후인소급조판단병정엄중정도，진이조기전면치료，가제고치유솔。
Objective To investigate and research clinical characteristics and prognostic influencing factor analysis for top of the basilar syndrome (TOBS). Methods A retrospective analysis was made on 52 patients with top of the basilar syndrome, and their clinical characteristics and prognostic influencing factors were analyzed. Results Top of the basilar syndrome had mainly clinical symptoms as disturbance of consciousness, headache, and dizziness, and its mainly clinical signs included ocular motility disorders, dyscoria, and acroparalysis. Adverse prognostic factors of top of the basilar syndrome included diabetes mellitus, hypertension, subtentorial, supratentorial complicated with subtentorial lesions>4, and Glasgow coma scale (GCS)≥10 points. Conclusion Clinical symptoms, signs and imaging performance are helpful for making early diagnosis and treatment for top of the basilar syndrome. Early judgment of severity by adverse prognostic factors ought to be made for applying early and comprehensive treatment and improving cure rate.
