目的：观察托伐普坦治疗顽固性心力衰竭合并低钠血症的临床效果。方法选取2013年1月~2014年6月在本院住院治疗的56例顽固性心力衰竭伴有低钠血症患者,将其随机分为治疗组和对照组,每组28例。对照组给予常规治疗,治疗组在常规治疗基础上给予托伐普坦,检测并比较两组治疗前后的血钠增加值、血渗透压、脑钠肽(BNP)及左室射血分数(LVEF)情况。结果治疗7 d后,两组的血钠、血渗透压、LVEF高于治疗前(P<0.05),BNP低于治疗前(P<0.05),且治疗组上述各项指标改善情况优于对照组,两组差异有统计学意义(P<0.05)。结论托伐普坦有助于升高血钠水平、改善心功能,在顽固性心力衰竭合并低钠血症的临床治疗中效果确切,值得临床推广应用。
목적：관찰탁벌보탄치료완고성심력쇠갈합병저납혈증적림상효과。방법선취2013년1월~2014년6월재본원주원치료적56례완고성심력쇠갈반유저납혈증환자,장기수궤분위치료조화대조조,매조28례。대조조급여상규치료,치료조재상규치료기출상급여탁벌보탄,검측병비교량조치료전후적혈납증가치、혈삼투압、뇌납태(BNP)급좌실사혈분수(LVEF)정황。결과치료7 d후,량조적혈납、혈삼투압、LVEF고우치료전(P<0.05),BNP저우치료전(P<0.05),차치료조상술각항지표개선정황우우대조조,량조차이유통계학의의(P<0.05)。결론탁벌보탄유조우승고혈납수평、개선심공능,재완고성심력쇠갈합병저납혈증적림상치료중효과학절,치득림상추엄응용。
Objective To observe the effect observation of tolvaptan in the treatment of refractory heart failure compli-cated with hyponatremia. Methods 56 patients with refractory heart failure with hyponatremia admitted into our hospi-tal from January 2013 to June 2014 were selected and randomly divided into treatment group and control group,with 28 cases in each group.The control group was given conventional treatment,on the basic of it,the treatment group was used tolvaptan.The increased blood sodium,blood osmolality,brain natriuretic peptide (BNP) and left ventricular ejection frac-tion (LVEF) was tested and compared before and after the treatment. Results 7 days after treatment,blood sodium,blood osmolality, LVEF in two groups was higher than before treatment,but BNP in two groups was lower than before treat-ment.The improvement of the above indexes in the treatment group was better than that in the control group,the differ-ence between two groups was significant (P<0.05). Conclusion Tolvaptan helped to increase the level of serum sodium, improve the heart function.It is effective in the treatment of refractory heart failure with hyponatremia,and is worthy of clinic application.
