目的：探讨彩色多普勒超声联合尿蛋白肌酐检测技术对高血压性早期肾损害患者的诊断价值。方法选取本院2012年2~8月的68例高血压性早期肾损害患者作为研究对象，均给予厄贝沙坦片和拜阿司匹林片治疗，并加用自拟“安泰固肾汤”和保肾康片，连续治疗1个月。所有受检者均进行尿蛋白/肌酐(UmAlb/UCr)及彩色多普勒超声检查。随访3个月后，比较彩色多普勒超声联合UmAlb/UCr检测与单纯尿蛋白肌酐检测两种检查方法的阳性率、准确度、灵敏度及特异度等。结果 UmAlb/UCr联合彩色多普勒检查的阳性率、准确度、灵敏度、特异度显著高于单纯性UmAlb/UCr检查，差异有统计学意义(P<0.05)。结论彩色多普勒超声联合尿蛋白肌酐检查检测技术对高血压性早期肾损害患者具有重要的诊断价值，值得临床推广应用。
목적：탐토채색다보륵초성연합뇨단백기항검측기술대고혈압성조기신손해환자적진단개치。방법선취본원2012년2~8월적68례고혈압성조기신손해환자작위연구대상，균급여액패사탄편화배아사필림편치료，병가용자의“안태고신탕”화보신강편，련속치료1개월。소유수검자균진행뇨단백/기항(UmAlb/UCr)급채색다보륵초성검사。수방3개월후，비교채색다보륵초성연합UmAlb/UCr검측여단순뇨단백기항검측량충검사방법적양성솔、준학도、령민도급특이도등。결과 UmAlb/UCr연합채색다보륵검사적양성솔、준학도、령민도、특이도현저고우단순성UmAlb/UCr검사，차이유통계학의의(P<0.05)。결론채색다보륵초성연합뇨단백기항검사검측기술대고혈압성조기신손해환자구유중요적진단개치，치득림상추엄응용。
Objective To explore the diagnostic value of color Doppler ultrasound combined with urinary protein/creatinine examination on patients with early renal injury in hypertension. Methods 68 cases with early renal injury in hypertension from February 2012 to August in our hospital were selected and given irbesartan tablets and aspirin tablets,Antai Gushen Decoction and Baoshenkang tablets were added,continuous treatment for 1 months.Urinary protein/creatinine (UmAlb/UCr),Doppler ultrasound examination were given to all the patients.After 3 months of follow-up,the positive rate,accuracy,sensitivity and specificity of UmAlb/UCr combined with Doppler ultrasound and UmAlb/UCr only was compared. Results The positive rate,accuracy,sensitivity and specificity of UmAlb/UCr combined with Doppler ultrasound examination was higher than that of UmAlb/UCr examination only,with significant difference(P<0.05). Conclusion Color Doppler ultrasound combined with urine protein/creatinine detection technology has important diagnostic value on patients with early renal injury in hypertension,it is worthy of clinical promotion and application.
