目的:探讨B超引导下应用塞丁格技术与常规行外周中心静脉置管术(PICC)对无菌性静脉炎的影响。方法75例行PICC术的乳腺癌患者,随机分为实验组和对照组,对照组(40例)行常规PICC置管,实验组(35例)于超声引导下应用塞丁格技术行PICC置管,观察两组行PICC术后无菌性静脉炎的发生率。结果实验组无菌静脉炎发生率为0,患者舒适度提高;对照组无菌静脉炎发生率为15%。两组无菌性静脉炎发生率差异有统计学意义(P<0.05)。结论无菌性静脉炎的发生与插管不畅、反复送管、携管期间插管侧上臂活动度过大、维护期敷料选择有关。超声引导下应用塞丁格技术行PICC置管,可以减少无菌性静脉炎的发生,提高置管成功率,增加患者舒适度,保证治疗有效进行。

Objective To investigate influence of B ultrasound-guided Seldinger technique and conventional peripherally inserted central catheter (PICC) on sterile phlebitis. Methods A total of 75 breast cancer patients receiving PICC were randomly divided into two groups, as control group (40 cases) received conventional PICC and experimental group (35 cases) received ultrasound-guided Seldinger technique for PICC. Incidences of sterile phlebitis after PICC were observed in the two groups. Results The experimental group had incidence of sterile phlebitis as 0, with high comfort degree in patients. Incidence of sterile phlebitis was 15%in the control group, and the difference had statistical significance between the two groups (P<0.05). Conclusion Occurrence of sterile phlebitis is related with blocked catheter, repeated intubation, over forearm activity during intubation, and dressing choice in maintenance period. Implement of ultrasound-guided Seldinger technique for PICC can reduce occurrence of sterile phlebitis, and improve success rate of intubation and patients’ comfort degree. It can guarantee effective progress in treatment.
