目的：探讨利用海博刀行膀胱肿瘤内镜黏膜下剥离术（BT‐ESD）治疗非肌层浸润性膀胱肿瘤的安全性和有效性。方法收集并分析2014年3月至2014年7月我院收治的28例利用海博刀行BT‐ESD治疗的膀胱肿瘤患者的临床资料。结果完整切除膀胱肿瘤28例，共42个独立病灶，肿瘤直径1．0～4．0 cm。手术时间（50．0±20．2）min ，膀胱冲洗时间（22±14）h ，尿管留置时间（2．1±1．7）d ，术后住院时间（3．7±2．3）d。无1例需要输血，未发生闭孔神经反射、穿孔及术后出血等并发症。结论利用海博刀行BT‐ESD治疗非肌层浸润性膀胱肿瘤是一种安全、有效的方式。肿瘤的完整切除有利于病理学的评估，减少了肿瘤播散和复发的机会。完整切除膀胱肿瘤的方法更符合肿瘤外科治疗原则。
목적：탐토이용해박도행방광종류내경점막하박리술（BT‐ESD）치료비기층침윤성방광종류적안전성화유효성。방법수집병분석2014년3월지2014년7월아원수치적28례이용해박도행BT‐ESD치료적방광종류환자적림상자료。결과완정절제방광종류28례，공42개독립병조，종류직경1．0～4．0 cm。수술시간（50．0±20．2）min ，방광충세시간（22±14）h ，뇨관류치시간（2．1±1．7）d ，술후주원시간（3．7±2．3）d。무1례수요수혈，미발생폐공신경반사、천공급술후출혈등병발증。결론이용해박도행BT‐ESD치료비기층침윤성방광종류시일충안전、유효적방식。종류적완정절제유리우병이학적평고，감소료종류파산화복발적궤회。완정절제방광종류적방법경부합종류외과치료원칙。
Objective To investigate the efficacy and safety of bladder tumor‐endoscopic sub‐mucosal dissection (BT‐ESD) for treatment of non‐muscle invasive bladder tumors by HybridKnife . Methods Forty‐two separate bladder tumors in 28 patients were dissected with BT‐ESD using Hy‐bridKnife from March 2014 to July 2014 . Results All the 42 lesions in the 28 patients were en bloc dis‐sected from the bladder wall . Operation time (50 .0 ± 20 .2 ) min , bladder irrigation time (22 ± 14) h ,indwelling catheter time (2 .1 ± 1 .7) d ,postoperative hospital stay (3 .7 ± 2 .3) d were found in these patients .No case needed blood transfusion .No obturator nerve reflex was observed and other complications occurred ,such as perforation and postoperative hemorrhage . Conclusions BT‐ESD with HybridKnife for treatment of non‐muscle invasive bladder tumor is a safe and effective approach .Complete resection of the tumor is conducive to evaluation of pathology ,reduction in the possibility of tumor spread and recurrence .The en‐bloc resection technique is more in line with the oncologic principles of surgery treatment of tumor .
