目的:探讨免拆式呼吸回路-湿化水罐在机械通气患儿中的应用效果。方法机械通气患儿共130例,观察组67例选用某品牌MR850型免拆式呼吸回路-湿化水罐,对照组63例选用传统呼吸机回路-湿化水罐。对组装步骤、接管时间、导管相关性肺炎发生率、意外脱管进行比较研究。结果两种回路使用比较显示,观察组管道组装时间为(25±5)秒,对照组为(70±10)秒,P<0.01;观察组意外脱管2例,脱管率为1.81%,对照组意外脱管6例,脱管率为5.61%,两组比较P<0.01,呼吸机相关性肺炎发生率观察组为8.96%,对照组为14.29%,两组比较P<0.05。结论免拆式呼吸回路-湿化水罐与传统呼吸回路-湿化水罐相比较,组装步骤少,接管时间短,在意外脱管率及导管相关性肺炎发生率方面更低。

목적:탐토면탁식호흡회로-습화수관재궤계통기환인중적응용효과。방법궤계통기환인공130례,관찰조67례선용모품패MR850형면탁식호흡회로-습화수관,대조조63례선용전통호흡궤회로-습화수관。대조장보취、접관시간、도관상관성폐염발생솔、의외탈관진행비교연구。결과량충회로사용비교현시,관찰조관도조장시간위(25±5)초,대조조위(70±10)초,P<0.01;관찰조의외탈관2례,탈관솔위1.81%,대조조의외탈관6례,탈관솔위5.61%,량조비교P<0.01,호흡궤상관성폐염발생솔관찰조위8.96%,대조조위14.29%,량조비교P<0.05。결론면탁식호흡회로-습화수관여전통호흡회로-습화수관상비교,조장보취소,접관시간단,재의외탈관솔급도관상관성폐염발생솔방면경저。
Objective To explore the application effect of disassembly free breathing circuit -humidification water tank in the children with mechanical ventilation.Methods A total of 130 cases of patients with mechanical ventilation, the 67 cases in observation group used a certain brand of MR850 type dismantle free breathing circuit-humidification water tank, and the other 63 cases in the control group adopted the traditional ventilator circuit-humidification water tank.The assembly steps, over time, ventilator-associated pneumonia occurrence rate and accident tube off of the two groups were compared.Results The pipe assembly time in observation group was 25 ±5seconds, and it was 70 ±10 seconds in the control group.2 cases of accidental decannulation occurred in observation group, decannulation rate was 1.81%, 6 cases of accidental decannulation occurred in control group, decannulation rate was 5.61%.The incidence of ventilator-associated pneumonia was 8.96% in the observation group, however, it was 14.29% in the control group.All the differences were statistically significant (P<0.01or 0.05).Conclusions The disassembly free breathing circuit-humidification water tank with less assembly steps and over time, lower accidental decannulation rate and ventilator-associated pneumonia incidence.
