目的:探讨白细胞介素(IL)17、IL-35、磷脂酶A2(PLA2)在急性冠状动脉综合征(ACS)患者血清中的水平变化及意义。方法选取2011年1月至2013年12月达州市中心医院收治的72例ACS患者为研究对象,ACS患者根据病情不同分为急性心肌梗死(AMI)组(46例),不稳定型心绞痛(UA)组(26例);另外选择同期收治的36例稳定型心绞痛(SA)患者(SA组)、52例健康人群(对照组)作为对照。统计受试者血清 IL-17、IL-35、PLA2水平,并对 ACS 患者血清 IL-17、IL-35、PLA2与ACS病情、年龄、烟龄、体质指数( BMI)进行相关性分析。结果 UA 组、AMI 组血清 IL-17水平[(17±5) ng/L、(15±5) ng/L]、PLA2[(167±10) ng/L、(166±10) ng/L]均高于 SA 组[(10±4) ng/L、(96±9) ng/L]、对照组[(9±4) ng/L、(90±8) ng/L](P<0.01);UA组、AMI组血清IL-35水平[(53±4) ng/L、(52±4) ng/L]均低于 SA 组[(61±5) ng/L]、对照组[(62±5) ng/L](P <0.01)。 ST段抬高组血清IL-17、PLA2水平均显著高于非ST段抬高组和UA组,血清IL-35水平显著低于非ST段抬高组和UA组( P<0.01)。 ACS患者 PCI术后血清 IL-17、IL-35、PLA2水平均较经皮冠状动脉介入术前显著降低( P <0.01)。血清 IL-17、PLA2水平与 ACS 病情、烟龄呈正相关,血清IL-35与ACS病情、烟龄呈负相关( P <0.01)。结论 IL-17、PLA2在 ACS 患者血清中呈高表达, IL-35在ACS患者血清中低表达;血清IL-17、IL-35、PLA2与 ACS发病及病情进展密切相关,可作为预测ACS发病及进展的指标。

목적:탐토백세포개소(IL)17、IL-35、린지매A2(PLA2)재급성관상동맥종합정(ACS)환자혈청중적수평변화급의의。방법선취2011년1월지2013년12월체주시중심의원수치적72례ACS환자위연구대상,ACS환자근거병정불동분위급성심기경사(AMI)조(46례),불은정형심교통(UA)조(26례);령외선택동기수치적36례은정형심교통(SA)환자(SA조)、52례건강인군(대조조)작위대조。통계수시자혈청 IL-17、IL-35、PLA2수평,병대 ACS 환자혈청 IL-17、IL-35、PLA2여ACS병정、년령、연령、체질지수( BMI)진행상관성분석。결과 UA 조、AMI 조혈청 IL-17수평[(17±5) ng/L、(15±5) ng/L]、PLA2[(167±10) ng/L、(166±10) ng/L]균고우 SA 조[(10±4) ng/L、(96±9) ng/L]、대조조[(9±4) ng/L、(90±8) ng/L](P<0.01);UA조、AMI조혈청IL-35수평[(53±4) ng/L、(52±4) ng/L]균저우 SA 조[(61±5) ng/L]、대조조[(62±5) ng/L](P <0.01)。 ST단태고조혈청IL-17、PLA2수평균현저고우비ST단태고조화UA조,혈청IL-35수평현저저우비ST단태고조화UA조( P<0.01)。 ACS환자 PCI술후혈청 IL-17、IL-35、PLA2수평균교경피관상동맥개입술전현저강저( P <0.01)。혈청 IL-17、PLA2수평여 ACS 병정、연령정정상관,혈청IL-35여ACS병정、연령정부상관( P <0.01)。결론 IL-17、PLA2재 ACS 환자혈청중정고표체, IL-35재ACS환자혈청중저표체;혈청IL-17、IL-35、PLA2여 ACS발병급병정진전밀절상관,가작위예측ACS발병급진전적지표。
Objective To study interleukin(IL)17,IL-35,phospholipase A2(PLA2) expression chan-ges and value in acute coronary syndrome(ACS).Methods Between Jan.2011 and Dec.2013,72 ACS patients from Dazhou Central Hospital were selected as the research objects,who according to the different conditions were divided into acute myocardial infarction group ( AMI group,46 cases) ,unstable angina group (UA group,26 cases).Another 36 patients with stable angina(SA group),52 healthy people(control group) during the same period were also included in the study.The serum IL-17,IL-35,PLA2 level were recorded, and the correlation between serum IL-17,IL-35,PLA2 and ACS disease,age,smoking history,BMI in pa-tients with ACS was analyzed.Results The serum level of IL-17 [(17 ±5) ng/L,(15 ±5) ng/L],PLA2 [(167 ±10) ng/L,(166 ±10) ng/L] of UA group,AMI group were higher than SA group [(10 ±4) ng/L, (96 ±9) ng/L],and the control group [(9 ±4) ng/L,(90 ±8) ng/L] (P<0.01); the serum level of IL-35 [(53 ±4),ng/L (52 ±4) ng/L] of UA group,AMI group were lower than the SA group [(61 ± 5) ng/L],and the control group [(62 ±5) ng/L](P <0.01).Serum IL-17,PLA2 levels of ST-elevation group were significantly higher than non-ST-elevation and UA group,serum IL-35 level of ST-elevation group was significantly lower than non-ST-elevation and UA group ( P <0.01 );the postoperative serum IL-17 , PLA2 levels of ACS patients with PCI were significantly higher than before PCI ,postoperative serum IL-35 level of ACS patients was significantly lower than before PCI(P<0.01);serum IL-17,PLA2 and ACS condi-tion,smoking age were positively correlated,serum IL-35 was negatively correlated with the ACS condition and smoking age(P<0.01).Conclusion IL-17,PLA2 present high expression in serum of ACS patients, IL-35 has low expression in patients with ACS;serum IL-17,IL-35,PLA2 are closely related to the onset and progression of ACS and they can be used as predictors of ACS onset and progression.
