目的:探讨外周血雌二醇、内脂素水平与绝经后女性OSA HS病情进展是否存在相关性。方法随机选择OSAHS绝经后女性患者40例和正常对照组(A组)20例,所有入选者均进行整夜7 h的多导睡眠呼吸监测,分为轻度(B组)20例和中重度(C组)20例。酶联免疫测定法(ELISA)测定空腹血清内脂素水平,光谱法测定雌二醇水平。并记录睡眠呼吸暂停低通气指数、睡眠呼吸障碍事件时最低SaO2及平均SaO2。分析各组间血清内脂素、雌二醇水平与睡眠监测指标的相关性和各组内血清内脂素与雌二醇的相关性。结果(1) B组及C组血清雌二醇水平均低于A组,且均与AHI呈负相关,与minSaO2、平均SaO2呈正相关。(2)C组血清内脂素较B组、A组显著升高,并与AHI呈正相关,与minSaO2、平均SaO2呈负相关;B组睡眠监测指标与血清内脂素无相关性。(3)不论B组还是C组,血清内脂素与雌二醇无相关性。结论绝经后女性OSAHS患者其血清雌二醇水平与病情严重程度呈负相关。存在肥胖的中重度绝经后女性OSA HS患者,血清内脂素水平与病情严重程度呈正相关。绝经后女性OSA HS患者的肥胖受血清雌二醇水平下降等多因素影响,可能是导致其血清雌二醇与内脂素无明显相关性的原因。

목적:탐토외주혈자이순、내지소수평여절경후녀성OSA HS병정진전시부존재상관성。방법수궤선택OSAHS절경후녀성환자40례화정상대조조(A조)20례,소유입선자균진행정야7 h적다도수면호흡감측,분위경도(B조)20례화중중도(C조)20례。매련면역측정법(ELISA)측정공복혈청내지소수평,광보법측정자이순수평。병기록수면호흡잠정저통기지수、수면호흡장애사건시최저SaO2급평균SaO2。분석각조간혈청내지소、자이순수평여수면감측지표적상관성화각조내혈청내지소여자이순적상관성。결과(1) B조급C조혈청자이순수평균저우A조,차균여AHI정부상관,여minSaO2、평균SaO2정정상관。(2)C조혈청내지소교B조、A조현저승고,병여AHI정정상관,여minSaO2、평균SaO2정부상관;B조수면감측지표여혈청내지소무상관성。(3)불론B조환시C조,혈청내지소여자이순무상관성。결론절경후녀성OSAHS환자기혈청자이순수평여병정엄중정도정부상관。존재비반적중중도절경후녀성OSA HS환자,혈청내지소수평여병정엄중정도정정상관。절경후녀성OSA HS환자적비반수혈청자이순수평하강등다인소영향,가능시도치기혈청자이순여내지소무명현상관성적원인。
Objective To discuss the serum estradiol levels ,whether a correlation exists between the progress of visfatin in obstructive sleep apnea hypopnea syndrome(OSAHS)in postmenopausal women .Method 20 postmenopa‐usal women was normal controls(Group A)and 40 postmenopausal women with OSAHS was experimental gruoup . Polysomnography in all subjects were all night 7h .Subjects in OSAHS group were divided into two groups according to apnea hypopnea index(AHI):Mild OSAHS group(group B)(5≤AHI<20 ,minSaO2≥86% )in 20 cases;moder‐ate to severe(group C)(20≤AHI ,minSaO2≤85% )in 20 cases .At the end of the monitoring of sleep apnea ,morning wake up 5 minutes fasting elbow venous blood 4ml preservation to be measured at -80 DEG C refrigerator .Adopt enzyme linked immunosorbent assay(ELISA)determination of fasting serum visfatin ,determination of estradiol spec‐trometry .The patients sleep monitoring process AHI ,minimum saturation level ,average blood oxygen saturation lev‐el .The correlation analysis between each group of serum visfatin ,estradiol and sleep monitoring index ,the correlation between serum visfatin and estradiol in each group .Result Mild OSAHS and moderate to severe OSAHS serum es‐tradiol levels were lower than the control group ,and were negatively correlated with AHI and positively correlated with minimum saturation level ,average blood oxygen saturation level .Moderate to severe OSAHS ,serum visfatin was more significant increased than the mild OSAHS and normal control group ,and was positively correlated with AHI and negatively correlated with minimum saturation level ,average blood oxygen saturation level .There were no correlation in mild OSAHS between sleep monitoring index and serum visfatin .Whether OSAHS patients with mild or moderate to severe ,serum visfatin had no correlation with estradiol .Conclusion In postmenopausal women with OSAHS ,negative correlation between the serum estradiol levels and severity of disease .There was widespread obesi‐ty in postmenopausal women with the severe OSAHS ,serum visfatin levels was positively correlated with severity of the disease .There were many factors lead to obesity for postmenopausal women with OSAHS ,such as serum estradi‐ol level decreased ,resulting in serum estradiol and visfatin has no obvious correlation .
