目的:通过两种桩核烤瓷冠修复方法,比较观察两种桩核临床效果的优劣及牙根的存在状况,为临床修复材料的选择提供依据。方法:选取患者88例(男48例,女40例),共88颗,因龋坏已致牙髓坏死的上前牙残冠。根的冠部均在龈缘以上,条件均等,随机分为两组,每组各44颗牙,分别行铸造金属桩核烤瓷冠和玻璃纤维桩核烤瓷冠修复,对修复体的远期效果进行评价。结果:术后6年,金属桩核组复查数36颗,27颗修复体保存完好,9颗修复体出现脱落,牙根出现不同程度的牙根折断。玻璃纤维桩核组复查数38颗,36颗修复体完好,功能良好。2颗出现玻璃纤维桩折断,但牙根完好,无松动,牙龈正常。两组成功率分别为75%和94.7%,两组差异有统计学意义(P<0.05)。结论:金属桩核组修复体的失败,根折是其主要原因。玻璃纤维组的失败,纤维桩折断是其主要因素。牙根的折断是不可复性的,纤维桩的折断,牙根还有二次修复的机会,因而纤维桩较金属桩更具有临床使用价值。

목적:통과량충장핵고자관수복방법,비교관찰량충장핵림상효과적우렬급아근적존재상황,위림상수복재료적선택제공의거。방법:선취환자88례(남48례,녀40례),공88과,인우배이치아수배사적상전아잔관。근적관부균재간연이상,조건균등,수궤분위량조,매조각44과아,분별행주조금속장핵고자관화파리섬유장핵고자관수복,대수복체적원기효과진행평개。결과:술후6년,금속장핵조복사수36과,27과수복체보존완호,9과수복체출현탈락,아근출현불동정도적아근절단。파리섬유장핵조복사수38과,36과수복체완호,공능량호。2과출현파리섬유장절단,단아근완호,무송동,아간정상。량조성공솔분별위75%화94.7%,량조차이유통계학의의(P<0.05)。결론:금속장핵조수복체적실패,근절시기주요원인。파리섬유조적실패,섬유장절단시기주요인소。아근적절단시불가복성적,섬유장적절단,아근환유이차수복적궤회,인이섬유장교금속장경구유림상사용개치。
Objective To compare the clinical effects of the residual crown and root restoration with the posts and cores of two materials to provide evidence for the choice of the right material in clinical treatment. Methods 48 male and 40 female patients with 88 residual crowns of carious upper incisors were included in the study.The tooth crowns were similar in condition and all above the gingival margin.They were randomly divided into two groups of 44 cases,restored using porcelain-veneered crowns with cast metal and glass fiber-reinforced posts and cores respectively.The long-term restoration effects were evaluated. Results The reexamination six years later of the 36 cases restored with cast metal posts and cores showed 27 crowns functioned well and 9 came off with varied root fracture.The reexamination of 38 cases restored with glass fiber-reinforced posts and cores revealed 36 crowns were in good shape and functioned well,and another 2 had broken glass fiber-reinforced posts.None of them reported root loosening or fracture.The restoration success rates of the two groups were 75% and 94.7%,respectively.Their difference had statistic significance(P<0.05). Conclusion The failure of the cast metal group consists mainly in the root fracture,while the failure of the glass fiber-reinforced group in the post breakage.The root fracture is irreparable whereas the post breakage leaves a chance for second restoration.So,glass fiber reinforced posts and cores have a higher clinical value than those cast metal ones.
