1例41岁慢性肾功能不全行血液透析的男性患者,因"发作性胸闷6 d"入院,诊断为冠状动脉粥样硬化性心脏病,不稳定型心绞痛.住院期间给予抗血小板、抗心肌缺血、强化降脂等治疗.临床药师重点关注了治疗冠心病药物对血液透析患者残余肾功能的影响.ACEI/ARB类药物可减缓血液透析患者残余肾功能的下降速度,对残余肾功能具有保护作用.而瑞舒伐他汀钙片因其肾损害的发生率较其他他汀类药物显著增高,建议血液透析患者避免使用.经治疗,患者病情好转,监测肾功能未见进一步降低.

One 41-year-old male patient with chronic renal inadequacy treated with hemodialysis was admitted into hospital because of onset of chest discomfort for 6 days. The diagnosis was coronary atherosclerotic heart disease, unstable angina pectoris. The patient was given antiplatelet drugs, anti-myocardial ischemia drugs and intensive lipid-lowering therapy in the hospitalization period. Clinical pharmacist paid more attention to the investigation of the drug effects on residual renal function. ACEI/ARB could slower the descending speed of residual renal function for the patient with hemodialysis, and had protective effects on it. The incidence of renal damage by rosuvastatin was signiifcantly higher than that of the other statins, which was suggested to avoid using on the patient with hemodialysis. The patient recovered well after treatment and renal function did not decrease.
