目的:探讨高危型人乳头瘤病毒(HPV)DNA检测在宫颈癌及癌前病变诊断中的作用。方法收集我院2011年~2013年妇科门诊宫颈癌筛查的1690例女性进行第2代杂交捕获法(HC-Ⅱ)检测HPV,以宫颈活检为金标准,评价高危型HPV-DNA检测在宫颈癌及癌前病变筛查中的作用。结果筛查并经病理确诊的宫颈癌及癌前病变237例,其中宫颈上皮内瘤变(CIN)Ⅰ级83例,Ⅱ级62例,Ⅲ级55例,浸润癌37例。高危型HPV-DNA检测宫颈癌及癌前病变的灵敏度、特异度、阳性预测值、阴性预测值分别为98.7%,17.1%,16.3%和98.8%。结论高危型HPV- DNA筛查宫颈癌具有很高的灵敏度和阴性预测值,是诊断宫颈癌及癌前病变的重要手段。

목적:탐토고위형인유두류병독(HPV)DNA검측재궁경암급암전병변진단중적작용。방법수집아원2011년~2013년부과문진궁경암사사적1690례녀성진행제2대잡교포획법(HC-Ⅱ)검측HPV,이궁경활검위금표준,평개고위형HPV-DNA검측재궁경암급암전병변사사중적작용。결과사사병경병리학진적궁경암급암전병변237례,기중궁경상피내류변(CIN)Ⅰ급83례,Ⅱ급62례,Ⅲ급55례,침윤암37례。고위형HPV-DNA검측궁경암급암전병변적령민도、특이도、양성예측치、음성예측치분별위98.7%,17.1%,16.3%화98.8%。결론고위형HPV- DNA사사궁경암구유흔고적령민도화음성예측치,시진단궁경암급암전병변적중요수단。
Objective To explore the effect of high risk human papillomavirus(HPV)DNA test on cervical cancer screening.Methods1690 women who were received cervical cancer screening in gynecological clinic in our hospital from 2011 to 2013 were detected HPV by hybrid-capture 2(HC-Ⅱ). Results Of cervical biopsy as gold standard, effects of high risk HPV-DNA test on screening cervical cancer and precancerous lesion were evaluated.Results 237 patients were screened and pathologically diagnosed with cervical cancer and precancerous lesion. Among which, there are 83 patients with cervical intraepithelial neoplasia (CIN)Ⅰ,62 patients with CINⅡ,55 patients with CIN Ⅲ and 37 with invasive carcinoma.The sensitivity,specificity,positive-predictive value and negative-predictive value of high risk HPV-DNA test for detecting cervical cancer and precancerous lesion were 98.7%,17.1%,16.3% and 98.8% respectively. ConclusionHigh risk HPV-DNA test has significantly high sensitivity and negative-predictive value which is an important method to detect cervical cancer and precancerous lesion.
