目的:探讨氨基末端脑钠肽(NT-proBNP)和高敏肌钙蛋白T(cTnT-hs)对老年社区获得性肺炎患者严重程度及预后的预测价值。方法对临床诊断为肺炎的156例老年患者评估肺炎严重指数(PSI),同时检测NT-proBNP和cTnT-hs。按照PSI分组,比较各组NT-proBNP和cTnT-hs水平,比较生存组与死亡组NT-proBNP和cTnT-hs水平,比较NT-proBNP和cTnT-hs双阳性组与NT-proBNP和cTnT-hs单阳性组间的病死率。结果各组NT-proBNP和cTnT-hs浓度随PSI分级而升高。死亡组患者中NT-proBNP浓度与存活组相比显著升高,差异有统计学意义(P<0.01)。NT-proBNP和cTnT-hs双阳性组与NT-proBNP和cTnT-hs单阳性组间病死率比较,差异具有统计学意义(P<O.01)。结论 NT-proBNP和cTnT-hs在评估老年社区获得性肺炎患者的病情严重程度及预后中具有重要作用,而联合检测血清NT-proBNP和cTnT-hs可更加有效地判断患者的预后。

Objective To evaluate the predictive value of combined measurement of N terminal B-type natriuretic peptide (NT-proBNP) and high-sensitivity cardiac troponin T (cTnT-hs) in elder community acquired pneumonia.Methods The blood levels of NT-proBNP and cTnT-hs in 156 patients with CAP were measured, the positive rates of cTnT-hs and NT-proBNP were also analyzed and compared between different groups. Results The blood levels of NT-proBNP and cTnT-hs increased with rising disease severity as classified by the PSI. NT-proBNP levels were signifi cantly higher in nonsurvivors compared to survivors (P<0.01). There was a signifi cant difference between the mortality in positive cTnT-hs alone and positive NT-proBNP alone group and both NT-proBNP and cTnT-hs positive group (P<0.01).Conclusion Either positive NT-proBNP or positive cTnT-hs plays an important role in evaluating the severity of diseases and predicting the prognosis in elder community acquired pneumonia. Combined measurement of NT-proBNP and cTnT-hs may help to assess prognosis more effective.
