目的:探讨呼吸康复治疗在慢性肺部疾病合并外科手术患者中应用研究方法。方法标准氧疗组(20例)和外科康复治疗组(22例)。康复组给予氧疗,同时配合呼吸康复训练治疗,标准氧疗组给予家庭氧疗。治疗前及治疗后分别测定患者的肺功能、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)和最大呼气流量(PEF),用mMRC评分、6-MWT试验、CAT评分、疼痛指数、下床时间等来评估两组患者肺功能、呼吸困难程度和日常活动能力的改善。结果干预结束后,慢性肺部疾病外科康复治疗组FVC、FEV1、改良的英国mMRC评分、6MWT试验、CAT评分等较标准氧疗组明显提高(P<0.05);慢性肺部疾病外科康复治疗组疼痛指数、下床时间、改良的英国mMRC评分也有明显改善(P<0.05)。结论呼吸康复训练配合氧疗能显著改善慢性肺部疾病合并外科手术患者的肺功能、运动耐力及呼吸困难、疼痛指数。

목적:탐토호흡강복치료재만성폐부질병합병외과수술환자중응용연구방법。방법표준양료조(20례)화외과강복치료조(22례)。강복조급여양료,동시배합호흡강복훈련치료,표준양료조급여가정양료。치료전급치료후분별측정환자적폐공능、용력폐활량(FVC)、제1초용력호기용적(FEV1)화최대호기류량(PEF),용mMRC평분、6-MWT시험、CAT평분、동통지수、하상시간등래평고량조환자폐공능、호흡곤난정도화일상활동능력적개선。결과간예결속후,만성폐부질병외과강복치료조FVC、FEV1、개량적영국mMRC평분、6MWT시험、CAT평분등교표준양료조명현제고(P<0.05);만성폐부질병외과강복치료조동통지수、하상시간、개량적영국mMRC평분야유명현개선(P<0.05)。결론호흡강복훈련배합양료능현저개선만성폐부질병합병외과수술환자적폐공능、운동내력급호흡곤난、동통지수。
ObjectiveTo explore research methods of application of pulmonary rehabilitation in patients with chronic lung disease combined with surgery.Methods 20 patients were in the standard oxygen treatment group and 22 patients were in the surgical rehabilitation treatment group. The surgical rehabilitation treatment group was given oxygen treatment accompanied by respiration rehabilitation training treatment while the standard oxygen treatment group was received home oxygen treatment. Pulmonary function (forced vital capacity) (FVC), first second of forced expiratory volume (FEV1) and peak expiratory flow (PEF) before and after treatment of patients were detected respectively. Pulmonary function, degree of dyspnea and improvement of daily activity of patients in two groups were accessed by mMRC score, 6MWT test, CAT score, pain index, getting out-of-bed time and so on. ResultsAfter intervention, FVC, FEV1, modified Britain mMRC score, 6-MWT test, CAT score and so on of the chronic lung disease combined with surgical rehabilitation treatment group were significantly improved when compared with the standard oxygen treatment group (P<0.05). Pain index, getting out-of-bed time and modified Britain mMRC score of the chronic lung disease combined with surgical rehabilitation treatment group were also significantly improved (P<0.05).ConclusionRespiration rehabilitation training treatment combined with oxygen treatment can significantly improve pulmonary function, exercise tolerance, dyspnea and pain index of patients with chronic lung disease combined with surgery.
