目的:探讨Destroscope虚拟现实系统在颅内动脉瘤诊疗中应用的价值。方法应用虚拟现实术前计划系统对20例颅内动脉瘤进行术前评估,观察解剖结构并进行手术模拟,同时结合术中情况与术前计划进行对比分析。结果20例颅内动脉瘤病例均成功地实现了颅骨及颅内血管的三维立体虚拟现实影像的重建。其中18例与计算机断层摄影血管造影术( computed tomographic angiography,CTA)报告结果一致,2例CTA报告可疑,在虚拟现实影像上清晰显示动脉瘤。20例患者均成功进行虚拟现实的手术模拟与手术设计,所得3D影像与手术所见一致。结论 Destroscope虚拟现实系统能够清楚地显露动脉瘤大小、位置,并可量化与周围结构的关系,有助于颅内动脉瘤的诊断及手术方案的选择,对提高手术安全性有意义。

목적:탐토Destroscope허의현실계통재로내동맥류진료중응용적개치。방법응용허의현실술전계화계통대20례로내동맥류진행술전평고,관찰해부결구병진행수술모의,동시결합술중정황여술전계화진행대비분석。결과20례로내동맥류병례균성공지실현료로골급로내혈관적삼유입체허의현실영상적중건。기중18례여계산궤단층섭영혈관조영술( computed tomographic angiography,CTA)보고결과일치,2례CTA보고가의,재허의현실영상상청석현시동맥류。20례환자균성공진행허의현실적수술모의여수술설계,소득3D영상여수술소견일치。결론 Destroscope허의현실계통능구청초지현로동맥류대소、위치,병가양화여주위결구적관계,유조우로내동맥류적진단급수술방안적선택,대제고수술안전성유의의。
Objective To investigate the value of Destroscope virtual reality system in the treatment of intracranial aneurysms. Method 20 cases of intracranial aneurysms were preoperative evaluated by using the virtual reality sys-tem,then the anatomic structure was observed and the operation simulation was carried out, and intraoperative con-ditionswith the plan before the operation was compared. Result Virtual reality three-dimensional images of the skull and intracranial vessels of the 20 artery aneurysm cases were successfully achieved. Among the results,18 cases were supported by the computed tomographic angiography ( CTA) results,2 cases were suspicious in CTA results,which-were finally diagnosed aneurysms on the virtual reality images. Surgical simulation and surgery program of virtual re-ality of the 20 patients were successfully performed. Conclusion Destroscope virtual reality system can clearly reveal the size,location of aneurysm,and quantify the relationships between aneurysm and surrounding structures,which is helpful to diagnose and select the best individualized rapeutic planning to improve outcome.
