目的::探讨心电图检测中QRS波群终末变形对判断急性心肌梗死( AMI)患者预后的临床意义。方法:将我院收治的AMI患者按照首次心电图诊断有无QRS波群终末变形分成A组和B组,经相同的治疗方式后,比较2组的预后。结果:A组吸烟史占75.0%,心绞痛史占62.5%,均明显高于B组的37.5%、25.0%(P<0.01),A组室性心律失常发生率47.0%,高于B组的18.8%(P<0.01),左室射血分数水平(43±12)%低于B组的(53±11)%(P<0.01)。结论:心电图检测中QRS波终末变形作为评估AMI预后的重要指标,可以对患者的预后情况进行更加科学准确的评估。

Objective:To explore the clinical significance of the QRS distortion of the terminal portion of ECG detection in determining the prognosis of patients with acute myocardial infarction(AMI). Methods:The AMI patients were divided into group A and group B according to the patients with the presence and absence of terminal QRS distortion in the first-time ECG diagnosis. The prognosis between two groups were compared after the same treatment. Results:Seventy-five percent patients with smoking history, 62. 5% of patients with angina pectoris history in A group were found,which was significantly higher than those in group B(37. 5% and 25.0%)(P<0.01).The incidence of ventricular arrhythmia in group A(47.0%) was higher than that in group B(18.8%)(P<0.01). The left ventricular ejection fraction in group A(43 ± 12)% was less than that in group B(53 ± 11)%(P <0. 01). Conclusions:The terminal QRS distortion in ECG diagnosis can be served as an important index in the evaluation of prognosis of AMI patients,which can make more scientific and accurate evaluation of prognosis.
