目的:评价覆膜支架置入治疗食管恶性狭窄的临床效果,并探讨术后并发症的相关因素和对策。方法:115例食管恶性狭窄患者在DSA下经口置入覆膜食管支架。支架使用国产支架,以16 mm直径支架为主。结果:手术成功率100%,共置入覆膜食管支架124枚。术后患者临床症状显著好转或消失。并发症包括严重疼痛2例(1.7%),再狭窄6例(5.2%),支架移位7例(6.0%),支架脱落2例(1.7%),严重贴覆不良2例(1.7%),支架端瘘1例(0.8%),支架腔嵌塞2例(1.7%),大出血1例(0.8%)。结论:覆膜支架置入治疗食管恶性狭窄是一种非常有效的方法,但是有一定风险。

목적:평개복막지가치입치료식관악성협착적림상효과,병탐토술후병발증적상관인소화대책。방법:115례식관악성협착환자재DSA하경구치입복막식관지가。지가사용국산지가,이16 mm직경지가위주。결과:수술성공솔100%,공치입복막식관지가124매。술후환자림상증상현저호전혹소실。병발증포괄엄중동통2례(1.7%),재협착6례(5.2%),지가이위7례(6.0%),지가탈락2례(1.7%),엄중첩복불량2례(1.7%),지가단루1례(0.8%),지가강감새2례(1.7%),대출혈1례(0.8%)。결론:복막지가치입치료식관악성협착시일충비상유효적방법,단시유일정풍험。
Objective: To evaluate the clinical efficacy of the covered stent placements in the treatment of malignant esophageal stenosis and analyze the interrelated factors and counter measures of complications.Method: 115 patients with malignant esophageal stenosis underwent treatment with the covered stent placements through the mouths under DSA fluoroscopy. The stents included China-made and the 16 mm stents were main choice.Result: The successful rate of operation was 100%, 124 covered stents were placed in total. The clinical symptoms of patients disappeared orabated obviously. Complications included: Severe pain in 2 cases (1.7%), restenosis in 6 cases (5.2%), stent migration in 7 cases (6.0%), stent fell off in 2 cases (1.7%), serious malapposition in 2 cases (1.7%), esophagus-mediasti-num fistula in 1 case (0.8%), stent jam in 2 cases (1.7%), massive hemorrhage in 1 case (0.8%).Conclusion: The covered stent placement in the treatment of malignant esophageal stenosis is a high effective method, but there is a certain risk.
