慢性心力衰竭是各种心脏疾病的终末阶段，其患病率不断上升，消耗了大量的医疗资源。除了规范的药物治疗以外，连续、规范、有效的管理显得至关重要。本文结合目前国内外常见的心力衰竭管理模式，并结合我科的实践，对于慢性心力衰竭的院外管理模式进行探讨。

Chronic heart failure is the end stage of various heart disease, the morbidity rate of which keep rising, consuming a lot of medical resources. Besides standard medication, continuous as well as effective management is of great importance. This article combined current management mode for heart failure at home and abroad as well as the clinical practice of our department, tried to investigate the management mode for chronic heart failure outside the hospital.
