目的：观察利多卡因复合芬太尼宫颈局部注射应用于人工流产术的麻醉效果。方法选取医院2012年9月-2014年11月收治的要求人工流产术的无麻醉、手术禁忌证的孕妇200例，随机分为A组和B组各100例。A组采用宫颈注射利多卡因复合芬太尼，B组采用宫颈注射利多卡因，观察2组镇痛效果，宫颈松弛效果、出血量、手术时间及不良反应发生率。结果 A组患者的镇痛有效率为98%高于B组的92%，手术时间短于B组，术后腹痛发生率低于B组，差异均有统计学意义（ P﹤0.01或P﹤0.05）。2组宫颈松弛、出血量比较差异无统计学意义（ P﹥0.05）。2组均未出现严重不良发应。结论利多卡因复合芬太尼宫颈局部麻醉用于人工流产术是安全、有效、价廉、简便的方法，值得临床推广应用。
목적：관찰리다잡인복합분태니궁경국부주사응용우인공유산술적마취효과。방법선취의원2012년9월-2014년11월수치적요구인공유산술적무마취、수술금기증적잉부200례，수궤분위A조화B조각100례。A조채용궁경주사리다잡인복합분태니，B조채용궁경주사리다잡인，관찰2조진통효과，궁경송이효과、출혈량、수술시간급불량반응발생솔。결과 A조환자적진통유효솔위98%고우B조적92%，수술시간단우B조，술후복통발생솔저우B조，차이균유통계학의의（ P﹤0.01혹P﹤0.05）。2조궁경송이、출혈량비교차이무통계학의의（ P﹥0.05）。2조균미출현엄중불량발응。결론리다잡인복합분태니궁경국부마취용우인공유산술시안전、유효、개렴、간편적방법，치득림상추엄응용。
Objective To explore the parturiency effect of lidocaine combined with fentanyl cervical local anesthetic in induced abortion. Methods selected 200 cases of pregnant women required induced abortion,from september 2012 to No-vember 2014 in the hospital,randomly divided into A group and B group( each of 100 cases). A group used cervical injection of lidocaine combined with fentanyl,B group used cervical injection of lidocaine. Observed the analgesic effect,cervical relaxation effects,blood loss,operation time and incidence of adverse reactions in two groups. Results The analgesic effective rate of A group was 98% higher than 92% of B group;The operation time of A group was shorter than B group;The incidence of postop-erative pain was lower than B group,the differences were statistically significant(P﹤0. 01 or P﹤0. 05). The difference of cer-vical relaxation,bleeding of two groups were not statistically significant( P ﹥0. 05 ). No serious adverse appearred in two groups. Conclusion Application of cervical local injection lidocaine combination fentanyl in induced abortion operation is safe,effective,low price and simple method. It is worth clinical application.
