目的：探讨管状吻合器用于乙状结肠造口术的优点及手术技巧。方法将73例行miles手术结肠永久性造口术的直肠癌患者随机分为2组，A组吻合器吻合肠造口39例，B组手工缝合肠造口34例。术后对2组造口方法从手术时间、术后造口排气时间、平均住院日、造口相关并发症发生率等方面进行统计学分析。结果2组手术均成功，无手术死亡病例。 A组手术时间、术后造口排气时间、术后平均住院日、造口肠管水肿发生率均低于B组，差异有统计学意义（P＜0．05）。结论经腹膜外行管状吻合器乙状结肠造口术操作简单，造口形态一致，手术时间明显减少，手术并发症少，是一种理想的手术方法，值得推广。
목적：탐토관상문합기용우을상결장조구술적우점급수술기교。방법장73례행miles수술결장영구성조구술적직장암환자수궤분위2조，A조문합기문합장조구39례，B조수공봉합장조구34례。술후대2조조구방법종수술시간、술후조구배기시간、평균주원일、조구상관병발증발생솔등방면진행통계학분석。결과2조수술균성공，무수술사망병례。 A조수술시간、술후조구배기시간、술후평균주원일、조구장관수종발생솔균저우B조，차이유통계학의의（P＜0．05）。결론경복막외행관상문합기을상결장조구술조작간단，조구형태일치，수술시간명현감소，수술병발증소，시일충이상적수술방법，치득추엄。
Objective To evaluate the clinical value and technique of circular anastomotic stapler for sigmoid colosto-my.Methods 73 cases of low colorectal cancer treated by miles operation were randomly divided into 2 groups.39 patients re-ceived sigmoid colostomy with circular anastomotic stapler ( group A) ,and 34 patients received sigmoid colostomy with conven-tional suturing techniques(group B).The medical records including operation time,time of bowl function,postoperative hospital stay and postoperative complications rate were analyzed statistically.Results All patients received the operations successfully.No operative death occurred in both groups.In group A,the operation time,time of bowl function,postoperative hospital stay time,and rate of edema of sigmoid were shorter than those of group B (P<0.05).Conclusion Circular anastomotic stapler fro extrperito-neal sigmoid colostomy is simple and effective,it creates good shape,shorten operation time,with fewer complications.It is worthy of popularization.
