冷凝集素综合征(CAS)是一种由于IgM抗体所引起的自身免疫性疾病，其特点是在正常体温以下这种抗体作用于自己的红细胞抗原发生红细胞凝集，致红细胞破坏增加而引起贫血，分为温抗体型、冷抗体型（冷凝集素综合征）、温冷抗体型，约占免疫性溶血性贫血的16%~32%[1]。CAS有急性和慢性两大类。慢性CAS以50~70岁中老年患者居多，寒冷环境下有耳廓、鼻尖、手指发绀，一经保暖加温即逐渐消失。主要症状有轻度或中度的慢性溶血性贫血和黄疸，寒冷天气可加重。天气寒冷时常因毛细血管中红细胞大量凝集而受阻发绀，因血管内溶血而出现血红蛋白尿和不同程度的贫血[2]。2014年2月5日我科收治1例胆囊炎合并冷凝集素综合征的患者，经过1个月的精心的治疗和保暖护理，取得满意效果。
냉응집소종합정(CAS)시일충유우IgM항체소인기적자신면역성질병，기특점시재정상체온이하저충항체작용우자기적홍세포항원발생홍세포응집，치홍세포파배증가이인기빈혈，분위온항체형、랭항체형（냉응집소종합정）、온랭항체형，약점면역성용혈성빈혈적16%~32%[1]。CAS유급성화만성량대류。만성CAS이50~70세중노년환자거다，한랭배경하유이곽、비첨、수지발감，일경보난가온즉축점소실。주요증상유경도혹중도적만성용혈성빈혈화황달，한랭천기가가중。천기한랭시상인모세혈관중홍세포대량응집이수조발감，인혈관내용혈이출현혈홍단백뇨화불동정도적빈혈[2]。2014년2월5일아과수치1례담낭염합병냉응집소종합정적환자，경과1개월적정심적치료화보난호리，취득만의효과。
Cold agglutinin comprehensive syndrome (CAS)is a due to an autoimmune disease caused by IgM antibody and its characteristics is in normal temperature below this antibody in their red blood cel antigen occurred agglutination of red blood cel s,causing red blood cel increased anti body,cold resistant type (cold agglutinin syndrome),warm and cold resistant type,accounting for about 16%of autoimmune hemolytic anemia~32%.CAS has two major categories of acute and chronic.Chronic CAS to 50~70 years mostly in elderly patients and cold environment with ear,nose,fingers, cyanosis,a warm heating gradual y disappear.The main symptoms are mild or moderate chronic hemolytic anemia and jaundice.The cold weather is often due to capil ary red cel agglutination and blocked cyanosis,due to intravascular hemolysis and hemoglobinuria and varying degrees of anemia.On 5 February 2014 in our department were 1 cases of cholecystitis with cold agglutinin syndrome patients,after a month of intensive treatment and warm care,and achieved satisfactory results.
