目的:探讨下颌第一磨牙近中根管的根管口分型和临床疗效。方法选取67颗进行根管治疗的下颌第一磨牙,观察并记录近中根管的根管口形态,采用机用镍钛器械及超声器械进行根管预备,热牙胶垂直加压充填技术充填根管;根据治疗前后临床检查及 X 线片评价根管预备和充填的效果。结果67颗牙中,双根管口发生概率最高,为61颗,占91.0%;三根管口次之,为4颗,占6.0%;单根管口出现率最低,为2颗(3.0%)。术后6个月复查X 线片评价根管治疗预后:痊愈35颗,有效25颗,无效7颗,治疗有效率为89.6%。结论下颌第一磨牙根管形态复杂,有必要更加详尽地了解牙根、根管的数目及其形态,超声冲洗和热牙胶垂直加压技术充填根管可获得良好的治疗效果。

목적:탐토하합제일마아근중근관적근관구분형화림상료효。방법선취67과진행근관치료적하합제일마아,관찰병기록근중근관적근관구형태,채용궤용얼태기계급초성기계진행근관예비,열아효수직가압충전기술충전근관;근거치료전후림상검사급 X 선편평개근관예비화충전적효과。결과67과아중,쌍근관구발생개솔최고,위61과,점91.0%;삼근관구차지,위4과,점6.0%;단근관구출현솔최저,위2과(3.0%)。술후6개월복사X 선편평개근관치료예후:전유35과,유효25과,무효7과,치료유효솔위89.6%。결론하합제일마아근관형태복잡,유필요경가상진지료해아근、근관적수목급기형태,초성충세화열아효수직가압기술충전근관가획득량호적치료효과。
Objective To investigate the root canal orifice typing of mesial canal of mandibular first molar and its clinical efficacy.Methods The morphology of root canal orifices of mesial ca-nals were observed and recorded in 67 mandibular first molars.Root canals were prepared with nickel-titanium instruments and ultrasonic instruments,and were obturated with warm gutta-per-cha using vertical condensation technique.Efficacies of root canal preparation and obturation were evaluated by clinical examination and X-radiographs.Results Among the 67 molars,61(91.0%) contained 2 orifices,4(6.0%)contained 3 orifices,and 2(3.0%)contained 1 orifice.At 6 months after treatment,X-radiographs showed that healing was achieved in 35 teeth,effective outcome in 25 teeth and ineffective outcome in 7 teeth.The rate of response to treatment was 89.6%.Conclu-sion The root canal of mandibular first molar shows complex morphology.Therefore,it is neces-sary to understand the number and morphology of tooth root and root canal.Ultrasonic irrigation and vertical condensation of warm gutta-percha can produce a good outcome.
