目的：：探讨检测血清透明质酸（HA）、III型前胶原N端肽（P IIIP N-P）、IV型胶原（C IV）、层粘连蛋白（LN）水平在评估肝纤维化程度及预后评估中的临床意义。方法：分别检测正常健康人群（A组50例）、急性肝炎患者（B组28例）、慢性病毒性肝炎患者（C组67例）、肝硬化患者（D组42例）、肝癌患者（E组13例）血清HA、P IIIP N-P、C IV、LN水平，比较各组间各指标差异及其变化趋势。结果：B组与A组比较各指标均无统计学差异（P＞0.05），C、D、E组与A组比较，各指标均明显升高（P＜0.05），且C组＜D组＜E组。结论：血清HA、PC-III、C IV、LN水平可客观反映肝纤维化程度，动态观察其变化可用于评估慢性肝病治疗效果。
목적：：탐토검측혈청투명질산（HA）、III형전효원N단태（P IIIP N-P）、IV형효원（C IV）、층점련단백（LN）수평재평고간섬유화정도급예후평고중적림상의의。방법：분별검측정상건강인군（A조50례）、급성간염환자（B조28례）、만성병독성간염환자（C조67례）、간경화환자（D조42례）、간암환자（E조13례）혈청HA、P IIIP N-P、C IV、LN수평，비교각조간각지표차이급기변화추세。결과：B조여A조비교각지표균무통계학차이（P＞0.05），C、D、E조여A조비교，각지표균명현승고（P＜0.05），차C조＜D조＜E조。결론：혈청HA、PC-III、C IV、LN수평가객관반영간섬유화정도，동태관찰기변화가용우평고만성간병치료효과。
Objective To detection the level of hyaluronic acid(HA), type III procollagen amino peptide(P IIIP N-P), collagen IV(C IV), laminin(LN) in serum, and to estimate severity and prognosis of liver fibrosis. Methods 50 health controls(Group A), 28 patients with acute hepatitis(Group B), 67 patients with chronic viral hepatitis(Group C), 42 patients with hepatic cirrhosis(Group D), 13 patients with liver cancer(Group E) were detected the level of HA, P IIIP N-P, C IV and LN in serum respectively, and to compare the variation between each groups. Results The level of HA, P IIIP N-P, C IV and LN between group A and group B have no statistic significance(P>0.05). However, the level of above indexes in group C, D and E were increased significantly than those in group A(P<0.05). Simultaneously, the levels were increased progressively from group C to group E. Conclusions The level of HA, P IIIP N-P, C IV and LN may reflect the severity of liver fibrosis objectively. Moreover, monitor the levels may be helpful to the evaluation of treatment effect of patients with chronic liver disease.
