目的：：明确皮肤共聚焦激光扫描显微镜( CLSM)在外阴部位硬化萎缩性苔藓与白癜风鉴别中的应用价值。方法：选取硬化萎缩性苔藓患者8例和白癜风患者16例的典型皮损做CLSM检查，然后组织病理学检查验证。结果：硬化萎缩性苔藓患者CLSM图像显示：表皮变薄、真表皮界线不清、真皮浅层折光度增加、单一核细胞浸润，与组织病理学特征一致。白癜风患者CLSM图像显示有白斑区色素完全缺失，或局部残留，与组织病理性特征一致。结论： CLSM有助于硬化萎缩性苔藓与白癜风的鉴别与诊断。
목적：：명학피부공취초격광소묘현미경( CLSM)재외음부위경화위축성태선여백전풍감별중적응용개치。방법：선취경화위축성태선환자8례화백전풍환자16례적전형피손주CLSM검사，연후조직병이학검사험증。결과：경화위축성태선환자CLSM도상현시：표피변박、진표피계선불청、진피천층절광도증가、단일핵세포침윤，여조직병이학특정일치。백전풍환자CLSM도상현시유백반구색소완전결실，혹국부잔류，여조직병이성특정일치。결론： CLSM유조우경화위축성태선여백전풍적감별여진단。
Objective: To determine the value of confocal laser scanning microscopy in identification be-tween atrophic lichen sclerosis and vitiligo on vulva. Methods: The lesions from 8 patients with atrophic li-chen sclerosis and 16 patients with vitiligo on vulva were examined under confocal laser scanning microscopy, and then, the histopathologic examination of the lesions was performed. Results:The image of atrophic lichen sclerosis lesions under CLSM showed that the thin epidermis, the obscured boundaries between dermis and ep-idermis, and the high diopter in the superficial layer of dermis and the infiltration of mononuclear cells. All a-bove was in accordance with the histopathologic features of atrophic lichen sclerosis. The image of vitiligo le-sions under CLSM showed melanin was completely or partly absent, which was in accordance with the his-topathologic features of vitiligo. Conclusion: CLSM is helpful in the differential diagnosis of atrophic lichen sclerosis and vitiligo.
