目的:探讨冠心病患者血浆中 microRNA-145(miRNA-145)表达与冠状动脉侧支循环(CCC)形成的相关性。方法根据冠状动脉造影结果入选80例主要冠状动脉中至少一支直径狭窄≥90%的冠心病患者,按照 Rentrop 分级方法对 CCC 进行分级,其中0级24例,1级19例,2级20例,3级17例。 Real-time PCR 测定血浆中 miRNA-145的相对表达,酶联免疫吸附法检测血浆中血管内皮生长因子(VEGF)水平,Gensini 积分方法评价冠状动脉病变程度。结果随着 CCC 分级的增加,血浆中 VEGF 水平亦随之升高,miRNA-145相对表达随之下降。控制 miRNA-145相对表达水平后,血浆中 VEGF 水平与 CCC 的建立呈正相关(r =0.524,P ﹤0.01),控制血浆中 VEGF 水平后,miRNA-145相对表达与 CCC 的建立呈负相关(r =-0.669,P ﹤0.01),同时 miRNA-145相对表达水平与血浆中VEGF 水平呈负相关(r =-0.529,P ﹤0.01)。结论在冠心病患者中,CCC 形成良好的患者血浆中miRNA-145相对表达低于 CCC 形成不良患者,表明 miRNA-145在 CCC 的建立中可能起重要作用。

목적:탐토관심병환자혈장중 microRNA-145(miRNA-145)표체여관상동맥측지순배(CCC)형성적상관성。방법근거관상동맥조영결과입선80례주요관상동맥중지소일지직경협착≥90%적관심병환자,안조 Rentrop 분급방법대 CCC 진행분급,기중0급24례,1급19례,2급20례,3급17례。 Real-time PCR 측정혈장중 miRNA-145적상대표체,매련면역흡부법검측혈장중혈관내피생장인자(VEGF)수평,Gensini 적분방법평개관상동맥병변정도。결과수착 CCC 분급적증가,혈장중 VEGF 수평역수지승고,miRNA-145상대표체수지하강。공제 miRNA-145상대표체수평후,혈장중 VEGF 수평여 CCC 적건립정정상관(r =0.524,P ﹤0.01),공제혈장중 VEGF 수평후,miRNA-145상대표체여 CCC 적건립정부상관(r =-0.669,P ﹤0.01),동시 miRNA-145상대표체수평여혈장중VEGF 수평정부상관(r =-0.529,P ﹤0.01)。결론재관심병환자중,CCC 형성량호적환자혈장중miRNA-145상대표체저우 CCC 형성불량환자,표명 miRNA-145재 CCC 적건립중가능기중요작용。
Objective To investigate the relationship between the expression of microRNA-145 (miRNA-145) in plasma and the formation of coronary collateral circulation ( CCC) in coronary artery disease patients. Methods A total of 80 patients with coronary stenosis were recruited. The CCC degree was rated by Rentrop rating system: rate 0 (n = 24) , rate 1 (n = 19) , rate 2 (n = 20) , rate 3 (n = 17). Plasma expression of miRNA-145 and concentration of VEGF were measured by real-time PCR and ELISA kit respectively. The severity of coronary artery atherosclerosis was evaluated by Gensini score system. Results The concentrations of VEGF in patients with coronary artery disease were increased according to CCC rating, whereas the expressions of miRNA-145 were decreased according to CCC rating. Correlation analysis demonstrated that there was a positive correlation between the plasma concentration of VEGF and CCC rating (r = 0. 672, P ﹤ 0. 01); while the expression of miRNA-145 in plasma showed a significantly negative correlation with the CCC rating and concentration of VEGF in those patients respectively (r = 0. 761,P ﹤0. 01, r = - 0. 529, P ﹤ 0. 01) . Conclusions In patients of coronary artery disease with better CCC formation, the expression of miRNA-145 in plasma is significantly lower than those with poor CCC formation, these findings may indicate that miRNA-145 may play an important role in the formation of CCC.
