目的::检测放疗前后Survivin、P53、Bcl-2和Bax在喉黏膜上皮组织中的表达,探讨其表达及与喉癌放疗敏感性的关系。方法:采用免疫组化SP法检测Survivin、P53、Bcl-2和Bax在56例单纯放疗的喉癌患者石蜡标本中的表达,同时用声带息肉正常喉黏膜组织石蜡块各10例进行对照,对所有患者进行1~5年的随访,同时检测放疗1年、3年、5年后上述四种癌基因蛋白表达情况与初次检测结果进行统计学分析。结果:Survivin、P53和Bcl-2在声带息肉组织中低表达,而Bax则呈相对高表达;Survivin、P53、Bcl-2在喉癌组织中呈高表达,而Bax则呈相对低表达。56例单纯放疗的喉癌患者中,放疗前及放疗1年、3年、5年后Survivin检出率分别是82.1%、67.1%、53.9%、32.8%。分析具有统计学意义(P<0.05)。P53放疗前的表达率76.7%,放疗1年、3年、5年后分别是54.6%、40.1%、26.8%,分析有统计学意义(P<0.05)。 Bcl-2放疗前为80.1%,放疗1年、3年、5年后分别为65.3%、42.1%、30.9%。分析有统计学意义(P<0.05)。 Bax放疗前52.4%,放疗1年、3年、5年后分别是49.3%、50.4%、47.7%,分析无统计学意义(P>0.05)。结论:Survivin、P53和Bcl-2在喉癌组组织中具有高表达,对放疗敏感的喉癌患者放疗后凋亡抑制基因Survivin、P53和Bcl-2呈现表达率降低的趋势,而Bax呈现表达率升高的趋势,将Survivin、P53、Bcl-2和Bax基因作为判断喉癌患者放疗敏感性的分子指标具有一定的临床运用价值。

Objective:To investigate the expression of Survivin,P53,Bax and Bcl-2 in laryngeal epithelium, and to investigate the relationship between the expression of laryngeal epithelium and the sensitivity to radiotherapy. Methods: The expression of Survivin group,P53,Bcl-2 and Bax expression were detected in 56 cases of laryngeal cancer patients with radiotherapy paraffin specimens, and laryngeal precancerous lesions such as paraffin blocks of leukoplakia of the larynx and vocal cord polyp of normal laryngeal mucosa in 10 cases were compared, combined with the clinical data, 1-5 year follow-up of all patients, the recurrence cases again the inspection organization, and detection of the four oncogene protein expression and initial test results and survival analysis was performed. Results:Survivin、P53、Bcl-2 showed low expression in vocal cord polyps, however,Bax showed a relatively high expression. Survivin, P53, Bcl-2 had a high expression in laryngeal carcinoma tissues, while Bax showed relatively low expression Of the 56 patients.Before radiotherapy,the rate is 82.1% ,one year after the radiotherapy,three years and five years later , the detection rate of Survivin in laryngeal carcinoma tissues was 67.1%,53.9% and 32.8%,respectively.Statistically significant difference (P < 0.05). The expression rate of P53 before radiotherapy is 76.7%, which is reduced to 54.6% after 1 year, 40.1% after 3 years,and 26.8% 5 years later which were significantly different (P < 0.05). Bcl-2 before radiotherapy was80.1%, after radiotherapy is 65.3%, 42.1%, 30.9%. Bax before radiotherapy is 52.4%,after radiotherapy it was respectively 49.3%, 50.4%, 47.7%, and there was no significant difference (P> 0.05). The expression of Survivin, P53, and Bcl-2 was positively correlated with the expression of Bax negatively. The positive cell rates of the expression of the four genes were correlated with the sensitivity of radiotherapy. Conclusion: the expression of Survivin, P53 and Bcl-2 in laryngeal cancer tissues with high expression, is sensitive to radiotherapy for laryngeal carcinoma patients with radiotherapy after inhibitor gene of apoptosis, Survivin, P53 and Bcl-2 showed the trend to reduce the rate of expression and Bax showed expression rates are on the rise trend, Survivin, P53, Bcl-2 and Bax gene as to judge the sensitivity of radiotherapy in patients with laryngeal cancer molecular marker has certain clinical application value.
